Pull SCSI updates from James Bottomley:
"Updates to the usual drivers (ufs, lpfc, qla2xxx, mpi3mr, libsas).
The major update (which causes a conflict with block, see below) is
Christoph removing the queue limits and their associated block
helpers.
The remaining patches are assorted minor fixes and deprecated function
updates plus a bit of constification"

Switch to the ->device_configure method instead of ->slave_alloc and update
the block limits on the passed in queue_limits instead of using the
per-limit accessors.
Note that uas was the only driver setting these size limits from
->slave_alloc and not ->slave_configure and this makes it match everyone
else.

In the scenario of entering hibernation with udisk in the system, if the
udisk was gone or resume fail in the thaw phase of hibernation. Its state
will be set to NOTATTACHED. At this point, usb_hub_wq was already freezed
and can't not handle disconnect event. Next, in the poweroff phase of
hibernation, SYNCHRONIZE_CACHE SCSI command will be sent to this udisk
when poweroff this scsi device, which will cause uas_submit_urbs to be
called to submit URB for sense/data/cmd pipe. However, these URBs will
submit fail as device was set to NOTATTACHED state. Then, uas_submit_urbs
will return a value SCSI_MLQUEUE_DEVICE_BUSY to the caller. That will lead
the SCSI layer go into an ugly loop and system fail to go into hibernation.
On the other hand, when we specially check for -ENODEV in function
uas_queuecommand_lck, returning DID_ERROR to SCSI layer will cause device
poweroff fail and system shutdown instead of entering hibernation.
To fix this issue, let uas_submit_urbs to return original generic error
when submitting URB failed. At the same time, we need to translate -ENODEV
to DID_NOT_CONNECT for the SCSI layer.

It has been observed that some USB/UAS devices return generic properties
hardcoded in firmware for mode pages for a period of time after a device
has been discovered. The reported properties are either garbage or they do
not accurately reflect the characteristics of the physical storage device
attached in the case of a bridge.
Prior to commit 1e029397d12f ("scsi: sd: Reorganize DIF/DIX code to
avoid calling revalidate twice") we would call revalidate several
times during device discovery. As a result, incorrect values would
eventually get replaced with ones accurately describing the attached
storage. When we did away with the redundant revalidate pass, several
cases were reported where devices reported nonsensical values or would
end up in write-protected state.
An initial attempt at addressing this issue involved introducing a
delayed second revalidate invocation. However, this approach still
left some devices reporting incorrect characteristics.
Tasos Sahanidis debugged the problem further and identified that
introducing a READ operation prior to MODE SENSE fixed the problem and that
it wasn't a timing issue. Issuing a READ appears to cause the devices to
update their state to reflect the actual properties of the storage
media. Device properties like vendor, model, and storage capacity appear to
be correctly reported from the get-go. It is unclear why these devices
defer populating the remaining characteristics.
Match the behavior of a well known commercial operating system and
trigger a READ operation prior to querying device characteristics to
force the device to populate the mode pages.
The additional READ is triggered by a flag set in the USB storage and
UAS drivers. We avoid issuing the READ for other transport classes
since some storage devices identify Linux through our particular
discovery command sequence.

There is usbdrv_wrap in struct usb_driver and usb_device_driver, it
contains device_driver and for_devices. for_devices is used to
distinguish between device drivers and interface drivers.
Like the is_usb_device(), it tests the type of the device. We can test
that if the probe of device_driver is equal to usb_probe_device in
is_usb_device_driver(), and then the struct usbdrv_wrap is no longer
needed.
Clean up struct usbdrv_wrap, use device_driver directly in struct
usb_driver and usb_device_driver. This makes the code cleaner.

DID_TARGET_FAILURE is internal to the SCSI layer. Drivers must not use it
because:
1. It's not propagated upwards, so SG IO/passthrough users will not see an
error and think a command was successful.
2. There is no handling for it in scsi_decide_disposition() so it results
in entering SCSI error handling.
It looks like the driver wanted a hard failure so this swaps it with
DID_BAD_TARGET which gives us that behavior. The error looks like it's for
a case where the target did not support a TMF we wanted to use (maybe not a
bad target but disappointing so close enough).

Set scsi_host_template.cmd_size instead of using the SCSI pointer for
storing driver-private data. Change the type of the argument of
uas_add_work() from struct uas_cmd_info * into struct scsi_cmnd * because
it is easier to convert a SCSI command pointer into a uas_cmd_info pointer
than the other way around.
This patch prepares for removal of the SCSI pointer from struct scsi_cmnd.

The DEF_SCSI_QCMD() macro passes the addresses of the SCSI host lock and
also that of the scsi_done function to the queuecommand_lck() function
implementations. Remove the 'scsi_done' argument since its address is
now a constant and instead call 'scsi_done' directly from inside the
queuecommand_lck() functions.

UAS does not share the pessimistic assumption storage is making that
devices cannot deal with WRITE_SAME. A few devices supported by UAS,
are reported to not deal well with WRITE_SAME. Those need a quirk.
Add it to the device that needs it.

The SCSI layer can go into an ugly loop if you ignore that a device is
gone. You need to report an error in the command rather than in the
return value of the queue method.
We need to specifically check for ENODEV. The issue goes back to the
introduction of the driver.

Use scsi_add_host_with_dma() instead of scsi_add_host().
When the scsi request queue is initialized/allocated, hw_max_sectors is clamped
to the dma max mapping size. Therefore, the correct device that should be used
for the clamping needs to be set.
The same clamping is still needed in uas as hw_max_sectors could be changed
there. The original clamping would be invalidated in such cases.

A SCSI error handler and block runtime PM must not allocate
memory with GFP_KERNEL. Furthermore they must not wait for
tasks allocating memory with GFP_KERNEL.
That means that they cannot share a workqueue with arbitrary tasks.
Fix this for UAS using a private workqueue.

When a uas disk is plugged into an external hub, uas_probe()
will be called by the hub thread to do the probe. It will
first create a SCSI host and then do the scan for this host.
During the scan, it will probe the LUN using SCSI INQUERY command
which will be packed in the URB and submitted to uas disk.
There might be a chance that this external hub with uas disk
attached is unplugged during the scan. In this case, uas driver
will fail to submit the URB (due to the NOTATTACHED state of uas
device) and try to put this SCSI command back to request queue
waiting for next chance to run.
In normal case, this cycle will terminate when hub thread gets
disconnection event and calls into uas_disconnect() accordingly.
But in this case, uas_disconnect() will not be called because
hub thread of external hub gets stuck waiting for the completion
of this SCSI command. A deadlock happened.
In this fix, uas will call scsi_scan_host() asynchronously to
avoid the blocking of hub thread.

Commit 3ae62a42090f ("UAS: fix alignment of scatter/gather segments"),
copying a similar commit for usb-storage, attempted to solve a problem
involving scatter-gather I/O and USB/IP by setting the
virt_boundary_mask for mass-storage devices.
However, it now turns out that the analogous change in usb-storage
interacted badly with commit 09324d32d2a0 ("block: force an unlimited
segment size on queues with a virt boundary"), which was added later.
A typical error message is:
ehci-pci 0000:00:13.2: swiotlb buffer is full (sz: 327680 bytes),
total 32768 (slots), used 97 (slots)
There is no longer any reason to keep the virt_boundary_mask setting
in the uas driver. It was needed in the first place only for
handling devices with a block size smaller than the maxpacket size and
where the host controller was not capable of fully general
scatter-gather operation (that is, able to merge two SG segments into
a single USB packet). But:
High-speed or slower connections never use a bulk maxpacket
value larger than 512;
The SCSI layer does not handle block devices with a block size
smaller than 512 bytes;
All the host controllers capable of SuperSpeed operation can
handle fully general SG;
Since commit ea44d190764b ("usbip: Implement SG support to
vhci-hcd and stub driver") was merged, the USB/IP driver can
also handle SG.
Therefore all supported device/controller combinations should be okay
with no need for any special virt_boundary_mask. So in order to head
off potential problems similar to those affecting usb-storage, this
patch reverts commit 3ae62a42090f.

Modules using these symbols are required to explicitly import the
namespace. This patch was generated with the following steps and serves
as a reference to use the symbol namespace feature:
1) Define DEFAULT_SYMBOL_NAMESPACE in the corresponding Makefile
2) make (see warnings during modpost about missing imports)
3) make nsdeps
Instead of a DEFAULT_SYMBOL_NAMESPACE definition, the EXPORT_SYMBOL_NS
variants can be used to explicitly specify the namespace. The advantage
of the method used here is that newly added symbols are automatically
exported and existing ones are exported without touching their
respective EXPORT_SYMBOL macro expansion.

This is the UAS version of

usb-storage: Set virt_boundary_mask to avoid SG overflows
We are not as likely to be vulnerable as storage, as it is unlikelier
that UAS is run over a controller without native support for SG,
but the issue exists.
The issue has been existing since the inception of the driver.

The same effects can be achieved by setting the dma_boundary to
PAGE_SIZE - 1 and the max_segment_size to PAGE_SIZE, so shift those
settings into the drivers. Note that in many cases the setting might
be bogus, but this keeps the status quo.

The ALWAYS_SYNC flag is currently honored by the usb-storage driver but not UAS
and is required to work around devices that become unstable upon being
queried for cache. This code is taken straight from:

Quoting Hans:
If we return 1 from our post_reset handler, then our disconnect handler
will be called immediately afterwards. Since pre_reset blocks all scsi
requests our disconnect handler will then hang in the scsi_remove_host
call.
This is esp. bad because our disconnect handler hanging for ever also
stops the USB subsys from enumerating any new USB devices, causes commands
like lsusb to hang, etc.
In practice this happens when unplugging some uas devices because the hub
code may see the device as needing a warm-reset and calls usb_reset_device
before seeing the disconnect. In this case uas_configure_endpoints fails
with -ENODEV. We do not want to print an error for this, so this commit
also silences the shost_printk for -ENODEV.
ENDQUOTE
However, if we do that we better drop any unconditional execution
and report to the SCSI subsystem that we have undergone a reset
but we are not operational now.

Now that the SPDX tag is in all USB files, that identifies the license
in a specific and legally-defined manner. So the extra GPL text wording
can be removed as it is no longer needed at all.
This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text. And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.
No copyright headers or other non-license-description text was removed.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.
Update the drivers/usb/ and include/linux/usb* files with the correct
SPDX license identifier based on the license text in the file itself.
The SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.

The uas driver has a subtle bug in the way it handles alternate
settings. The uas_find_uas_alt_setting() routine returns an
altsetting value (the bAlternateSetting number in the descriptor), but
uas_use_uas_driver() then treats that value as an index to the
intf->altsetting array, which it isn't.
Normally this doesn't cause any problems because the various
alternate settings have bAlternateSetting values 0, 1, 2, ..., so the
value is equal to the index in the array. But this is not guaranteed,
and Andrey Konovalov used the syzkaller fuzzer with KASAN to get a
slab-out-of-bounds error by violating this assumption.
This patch fixes the bug by making uas_find_uas_alt_setting() return a
pointer to the altsetting entry rather than either the value or the
index. Pointers are less subject to misinterpretation.

Commit 198de51dbc34 ("USB: uas: Limit qdepth at the scsi-host level")
removed the scsi_change_queue_depth() call from uas_slave_configure()
assuming that the slave would inherit the host's queue_depth, which
that commit sets to the same value.
This is incorrect, without the scsi_change_queue_depth() call the slave's
queue_depth defaults to 1, introducing a performance regression.
This commit restores the call, fixing the performance regression.

Commit 64d513ac31bd ("scsi: use host wide tags by default") causes
the SCSI core to queue more commands then we can handle on devices with
multiple LUNs, limit the queue depth at the scsi-host level instead of
per slave to fix this.

The uas driver can never queue more then MAX_CMNDS (- 1) tags and tags
are shared between luns, so there is no need to claim that we can_queue
some random large number.
Not claiming that we can_queue 65536 commands, fixes the uas driver
failing to initialize while allocating the tag map with a "Page allocation
failure (order 7)" error on systems which have been running for a while
and thus have fragmented memory.

Some devices send response IUs when you'd expect a sense IU.
As a response to a wrong LUN that is within spec.
We cannot get away without handling for response IUs.

This patch changes the !blk-mq path to the same defaults as the blk-mq
I/O path by always enabling block tagging, and always using host wide
tags. We've had blk-mq available for a few releases so bugs with
this mode should have been ironed out, and this ensures we get better
coverage of over tagging setup over different configs.

The usb-storage driver sets max_sectors = 240 in its scsi-host template,
for uas we do not want to do that for all devices, but testing has shown
that some devices need it.
This commit adds a US_FL_MAX_SECTORS_240 flag for such devices, and
implements support for it in uas.c, while at it it also adds support
for US_FL_MAX_SECTORS_64 to uas.c.

uas_use_uas_driver may set some US_FL_foo flags during detection, currently
these are stored in a local variable and then throw away, but these may be
of interest to the caller, so add an extra parameter to (optionally) return
the detected flags, and use this in the uas driver.

Drop the now unused reason argument from the ->change_queue_depth method.
Also add a return value to scsi_adjust_queue_depth, and rename it to

Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth. For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.
Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway. The one other case looks
broken, but I've kept the behavior as-is for now.
Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.
Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

Allow a driver to ask for block layer tags by setting .use_blk_tags in the
host template, in which case it will always see a valid value in
request->tag, similar to the behavior when using blk-mq. This means even
SCSI "untagged" commands will now have a tag, which is especially useful
when using a host-wide tag map.

Currently scsi piggy backs on the block layer to define the concept
of a tagged command. But we want to be able to have block-level host-wide
tags assigned even for untagged commands like the initial INQUIRY, so add
a new SCSI-level flag for commands that are tagged at the scsi level, so
that even commands without that set can have tags assigned to them. Note
that this alredy is the case for the blk-mq code path, and this just lets
the old path catch up with it.
We also set this flag based upon sdev->simple_tags instead of the block
queue flag, so that it is entirely independent of the block layer tagging,
and thus always correct even if a driver doesn't use block level tagging
yet.
Also remove the old blk_rq_tagged; it was only used by SCSI drivers, and
removing it forces them to look for the proper replacement.

With uas over usb-3 the tags inside the uas iu-s must match the usb-3 stream
ids, and those go from 1 - qdepth.
Before blk-mq calling scsi_activate_tcq(sdev, qdepth) guaranteed that we would
only get cmnd->request->tag from 0 - (qdepth - 1), and we used those as
uas-tags / stream-ids.
With blk-mq however we are guaranteed to never get more then qdepth commands
queued at the same time, but the cmnd->request->tag values may be much larger,
which breaks uas.
This commit fixes this by generating uas tags in the 1 - qdepth range ourselves
instead of using cmnd->request->tag.
While touching all involved code anyways also rename the uas_cmd_info stream
field to uas_tag, because when using uas over usb-2 streams are not used.

The uas driver uses the block layer tag for USB3 stream IDs. With
blk-mq we can get larger tag numbers that the queue depth, which breaks
this assumption. A fix is under way for 3.18, but sits on top of
large changes so can't easily be backported. Set the disable_blk_mq
path so that a uas device can't easily crash the system when using
blk-mq for SCSI.

If something goes wrong in our communication with an uas device we may get
a response iu in reaction to a cmnd, rather then a status iu. In this case
propagate an error upwards, rather then logging a bogus iu message.

Instead of doing:

On error, resulting in 2 log calls for a single error, make uas_log_cmd_state
take a status code, and change calls like the above to:
uas_log_cmd_state(cmnd, "error doing foo", err)
Also change various sanity checks (which should never trigger) from:
"scmd_printk(KERN_ERR, cmnd, "sanity foo failed\n")" to calling the new
uas_log_cmd_state(), so that when they do trigger we get more info.

I've access to a number of different uas devices now, and none of them use
old style sense urbs. The only case where these code-paths trigger is with
the asm1051 and there they do the wrong thing, as the asm1051 sends 8 bytes
status iu-s when it does not have any sense data, but uses new style
sense iu-s regardless, as can be seen for scsi cmnds where there is sense
data.

It was only used to sanity check against completing the same cmnd twice,
but that is the case we're likely operating on free-ed memory, and doing
sanity checks on free-ed memory is not really helpful.

The purpose of uas_pre_reset is to:
1) Stop any new commands from being submitted while an externally triggered
usb-device-reset is running
2) Wait for any pending commands to finish before allowing the usb-device-reset
to continue
The purpose of uas_suspend is to:
2) Wait for any pending commands to finish before suspending
This commit fixes races in both paths:
1) For 1) we use scsi_block_requests, but the scsi midlayer calls queuecommand
without holding any locks, so a queuecommand may already past the midlayer
scsi_block_requests checks when we call it, add a check to uas_queuecommand
to fix this
2) For 2) we were waiting for all sense-urbs to complete, there are 2 problems
with this approach:
a) data-urbs may complete after the sense urb, so we need to check for those
too
b) if a sense-urb completes with a iu id of READ/WRITE_READY a command is not
yet done. We submit a new sense-urb immediately in this case, but that
submit may fail (in which case it will get retried by uas_do_work), if this
happens the sense_urbs anchor may become empty while the cmnd is not yet
done
Also unblock requests on timeout, to avoid things getting stuck in that case.

Not all urbs we've allocated are necessarily also submitted, non-submitted
urbs will not be free-ed by their completion handler. So we need to free
them manually.
There are 2 scenarios where this can happen:
1) We have failed to submit some urbs at abort / disconnect
2) When running over usb-2 we may have never tried to submit the data urbs
when completing the scsi cmnd, because we never got a READ/WRITE_READY iu

The data urbs are all killed before calling zap_pending, and their completion
handler should have cleared their inflight flag.
Do not 0 the data inflight flags, and add a check for try_complete succeeding,
as it should always succeed when called from zap_pending.

Drop the whole dance with first moving cmnds to a dead-list. The resetting
flag ensures that no new cmds / urbs will be submitted, and that any urb
completions are short-circuited without trying to complete the scsi cmnd.

There is no need for all the trickery with dropping the lock, we can
simply reference the urbs while we hold the lock to ensure the urbs don't
disappear beneath us, and do the actual unlink (+ unreference) after we've
dropped the lock.
This also fixes a race where we may loose of cmnd ownership to the scsi
midlayer without holding the lock due to the midlayer re-claiming ownership
through an abort (which will be handled by a future patch in this series).

should we get a second status urb for the same tag after a IU_ID_STATUS.

Using scsi_host_find_tag with tags returned by the device is unsafe for
multiple reasons:
1) It returns tags->rqs[tag], which may be non NULL even when the cmnd is
not owned by us
2) It returns tags->rqs[tag], without holding any locks protecting it
3) It returns tags->rqs[tag], without doing any boundary checking
Instead keep our own list which maps tags -> inflight cmnds.

There are various bug reports about oopses / hangs with the uas driver,
which all point to the abort-command and logical-unit-reset (task-management)
error handling paths.
Getting these right is very hard, there are quite a few corner cases, and
testing is almost impossible since under normal operation these code paths
are not used at all.
Another problem is that there are also some cases where it simply is not clear
what to do at all. E.g. over usb-2 multiple outstanding commands share the same
endpoint. What if a command gets aborted while its sense urb is half way
through completing (so some data has been transfered but not all). Since the
urb is not yet complete we don't know if the sense urb is actually for this
command, or for one of the other oustanding commands. If it is for one of the
other commands and we cancel it, then we end up in an undefined state. But if
it is actually for the command we're aborting, and the abort succeeds, then it
may never complete...
This exact same problem applies to logical unit resets too, if there are
multiple luns, then commands outstanding on both luns share the sense
endpoint. If there is only a single lun, then doing a logical unit reset is
little better then doing a full usb device reset.
So summarizing because:
1) abort / lun-reset is very tricky to get right
2) Not being able to test the tricky code, which means it will have bugs
3) This being a code path which under normal operation will never happen,
so being slow / sub-optimal here is not really an issue
4) Under error conditions we will still be able to recover through usb
device resets.
5) This may be a bit slower in some cases, but this is actually faster in
cases where the bridge ship has locked up, which seems to be the most
common error case sofar.
This commit removes the abort / lun-reset error handling paths, and also the
taks-mgmt code since those are the only 2 task-mgmt users. Leaving only the
(tested and testable) usb-device-reset error handling path in place.
Note I realize that this is somewhat of a big hammer, but currently people
are seeing very hard to debug oopses with uas. First let focus on making uas
work reliable, then we can later look into adding more fine grained error
handling.

Besides the ASM1051 (*) needing sdev->no_report_opcodes = 1, it turns out that
the JMicron JMS567 also needs it to work properly with uas (usb-storage always
sets it). Since some of the scsi devs were not to keen on the idea to
outrightly set sdev->no_report_opcodes = 1 for all uas devices, so add a quirk
for this, and set it for the JMS567.
*) Which has become a non-issue since we've completely blacklisted uas on
the ASM1051 for other reasons

Some jmicron uas chipsets act up (they disconnect from the bus) when sending
more then 32 commands to them at once.
Rather then building an ever growing list with usb-id based quirks for
devices using this chipset, simply reduce the qdepth to 32 when connected
over usb-2. 32 should be plenty to keep things close to maximum
possible throughput on usb-2.

intfdata is set only after scsi_scan(). uas_pre_reset() however
needs intfdata to be valid and will follow the NULL pointer
killing khubd. intfdata must be preemptively set before the
host is registered and undone in the error case.

Although an interesting concept, I don't think that this is a good idea:
-This will result in lots of "virtual" scsi controllers confusing users
-If we get a scsi-bus-reset we will now need to do a usb-device-reset of all
uas devices on the same usb bus, which is something to avoid if possible

The scsi error handling path re-uses previously queued up (and errored-out)
cmds. If such a re-used cmd had a data-phase then cmdinfo will have
data_in_urb / data_out_urb still set to the free-ed urbs from the errored-out
cmd, and they will get free-ed a second time when the error handling cmd
completes, corrupting the kernel heap.
Clearing cmdinfo on command queue-ing fixes this, and seems like a good idea
in general.

The scsi-host structure is refcounted, scsi_remove_host tears down the
scsi-host but does not decrement the refcount, so we need to call
scsi_put_host on disconnect to get the underlying memory to be freed.
After calling scsi_remove_host, the scsi-core may still hold a reference to
the scsi-host, iow we may still get called after uas_disconnect, but we
do our own life cycle management of uas_devinfo, freeing it on disconnect,
and thus may end up using devinfo after it has been freed. Switch to letting
scsi_host_alloc allocate and manage the memory for us.

Before this commit the uas driver would keep track of scsi commands which still
need to have some urbs submitted to the device, and complete this with an
ABORT result code on bus-reset or disconnect, but in flight scsi commands
which have all their urbs submitted, and thus are not part of the work list,
would never get their done callback called.
The problem is killed sense urbs don't have any tag info, so it is impossible
to tell which scsi cmd they belong to, so merely making sure all the urbs
have completed one way or the other is not enough.
This commit fixes this by changing the work list to an inflight list, which
keeps tracks of all inflight scsi cmnds, using the IS_IN_WORK_LIST flag to
determine if actual work needs to be done in uas_do_work(), and by moving
marking all inflight scsi commands as aborted and moving them to the dead list
on bus-reset or disconnect.

Some BIOS-es will hang on reboot when an uas device is attached and left in
uas mode on reboot.
This commit adds a shutdown handler which on reboot puts the device back into
usb-storage mode, fixing the hang on reboot on these systems.

I thought it would be a good idea to also test uas with usb-2, and it turns out
it was, as it did not work. The problem is that the uas driver was passing the
bEndpointAddress' direction bit to usb_rcvbulkpipe, the xhci code seems to not
care about this, but with the ehci code this causes usb_submit_urb failure.
With this fixed the uas code works nicely with an uas device plugged into
an ehci port.

All callers of unlink_data_urbs drop devinfo->lock before calling it, and
then immediately take it again after the call. And the first thing
unlink_data_urbs does is take the lock again, and the last thing it does
is drop it. This commit removes all the unnecessary lock dropping and taking.

Otherwise they may complete before they get anchored and thus never get
unanchored (as the unanchoring is done by the usb core on completion).
This commit also remove the usb_get_urb / usb_put_urb around cmd submission +
anchoring, since if done in the proper order this is not necessary.

This patch adds a new list where all requests which are canceled are
added to, so we don't loose them. Then, after killing all inflight
urbs on bus reset (and disconnect) we'll walk over the list and clean
them up.
Without this we can end up with aborted requests lingering around in
case of status pipe transfer errors.

Simplifies locking, we'll protect the list with the device spin lock.
Also plugs races which can happen when two devices operate on the
global list.
While being at it rename the list head from "list" to "work", preparing
for the addition of a second list.

Add new function to unlink and abort requests from the work
list, call it on bus reset and disconnect where we kill all
in-flight urbs. Also reorder calls in disconnect to first
cancel transfers, then remove the scsi hba.

Two changes. First we check whenever the request is linked in the work
list and if so take it out. Second check whenever the command is
actually in flight before asking the device to cancel it via task
management, and in case it isn't just zap the data urbs and finish it.

Set state bits after submitting data urbs & command urbs, so we know
what is in flight. Clear data bits when the data urb is finished, clear
command bit when we see the status urb for the command. Finish the scsi
command after running both status and data completion handlers for the
command.
Add a cmd status logging function for debugging purposes. Hook it into
the error handler, so we see in the log what status a command is in
which the scsi layer wants cancel.

Stop reusing sense urbs, just allocate a fresh one each time and free it
when done.
Stop storing a sense urb pointer in the scsi request, all you can do
with it is misusing. For example requeuing the sense urb, then f*ck it
up by picking the wrong one in case tagged requests don't finish in the
same order you've submitted them. Also note that (not-yet supported)
task management ops don't have a scsi request in the first place.

IMO the real bug is assigning status urbs to scsi requests. First there
is no such link in the non-stream case. Also there isn't nessesarely a
scsi request in the first place, for example when submitting task
management requests.
This patch just papers over the real bug and introduces different status
urb handling in the stream/non-stream case for no good reason.

This patch makes uas.c call usb_unlink_urb on data urbs. The data urbs
get freed in the completion callback. This is illegal according to the
usb_unlink_urb documentation.
This patch also makes the code expect the data completion callback
being called before the status completion callback. This isn't
guaranteed to be the case, even though the actual data transfer should
be finished by the time the status is received.
Background: The ehci irq handler for example only know that there are
finished transfers, it then has go check the QHs & TDs to see which
transfers did actually finish. It has no way to figure in which order
the transfers did complete. The xhci driver can call the callbacks in
completion order thanks to the event queue. This does nicely explain
why the driver is solid on a (usb2) xhci port whereas it goes crazy on
ehci in my testing.

Just run into the following:
- new disk arrived in the system
- udev couldn't wait to get its hands on to to run ata_id /dev/sda
- this sent the cdb 0xa1 to the device.
- my UAS-gadget recevied the cdb and had no idea what to do with it. It
decided to send a status URB back with sense set to invalid opcode.
- the host side received it status and completed the scsi command.
- the host sent another scsi with 4kib data buffer
- Now I was confused why the data transfer is only 512 bytes instead of
4kib since the host is always allocating the complete transfer in one
go.
- Finally the system crashed while walking through the sg list.
This patch adds three new flags in order to distinguish between DATA
URB completed and outstanding. If we receive status before data, we
cancel data and let data complete the command.
This solves the problem for IN and OUT transfers but does not work for
BIDI.

The UAS driver requires SG support by the HCD operating the device. This
patch stops UAS from operating on a HCD without sg support and prints a
message to let him know.
The spec says:
|For [USB2] backward compatibility, the device shall present [BOT] as
|alternate interface zero (primary) and [UAS] as alternate interface one
|(secondary). A device which does not need backward compatibility with
|[BOT] shall present [UAS] as alternate interface zero. In [USB2]
|systems, the [BOT] driver or an associated filter driver may need to
|issue a SET INTERFACE request for alternate interface one and then allow
|the [UAS] driver to load.
If the user used usb_modeswitch to switch to UAS then he can go back to
BOT or use a different HCD. In case UAS is the only interface then there
is currently no way out.
In future usb_sg_wait() should be extended to provide a non-blocking
interface so it can work with the UAS driver.

The status/sense URB is allocated on per-command basis. A read/write
looks the following way on a stream-less connection:
- send cmd tag X, queue status
- receive status, oh it is a read for tag X. queue status & read
- receive read
- receive status, oh I'm done for tag X. Cool call complete and free
status urb.
This block repeats itself 1:1 for further commands and looks great so
far. Lets take a look now what happens if we do allow multiple commands:
- send cmd tag X, queue statusX (belongs to the command with the X tag)
- send cmd tag Y, queue statusY (belongs to the command with the Y tag)
- receive statusX, oh it is a read for tag X. queue statusX & a read
- receive read
- receive statusY, oh I'm done for tag X. Cool call complete and free statusY.
- receive statusX, oh it is a read for tag Y. queue statusY & before we
queue the read the the following message can be observed:
|sd 0:0:0:0: [sda] sense urb submission failure
followed by a second attempt with the same result.
In order to address this problem we will use only one status URB for
each scsi host in case we don't have stream support (as suggested by
Matthew). This URB is requeued until the device removed. Nothing changes
on stream based endpoints.

In "usb/uas: use unique tags for all LUNs" we make sure to create unique
tags across all LUNs. This patch uses scsi_host_find_tag() to obtain the
correct command which is associated with the tag.
The following changes are required:
- don't use sdev->current_cmnd anymore
Since we can have devices which don't support command queueing we must
ensure that we can tell the two commands apart. Even if a device
supports comand queuing we send the INQUIRY command "untagged" for
LUN1 while we can send a tagged command to LUN0 at the same time.
devinfo->cmnd is used for stashing the one "untagged" command.
- tag number is altered. If stream support is used then the tag number
must match the stream number. Therefore we can't use tag 0 and must
start at tag 1.
In case we have untagged commands (at least the first command) we must
be able to distinguish between command tag 0 (which becomes 1) and
untagged command (which becomes curently also 1).
The following tag numbers are used:
0: never
1: for untagged commands (devinfo->cmnd)
2+: tagged commands.

I observed that on a device with multiple LUNs UAS was re-using the same
tag number for requests which were issued at the same time to both LUNs.
This patch uses scsi_init_shared_tag_map() to use unique tags for all
LUNs. With this patch I haven't seen the same tag number during the init
sequence anymore. Tag 1 is used for devices which do not adverise
command queueing.
This patch initilizes the queue before adding the scsi host like the
other two user in tree.

In the UAS status URB completion handler, we need to free the URB, no
matter what happens. Fix a bug where we would leak the URB (and its
buffer) if we couldn't find a SCSI command that is associated with this
status phase.

UAS can work with either USB 3.0 devices that support bulk streams, or
USB 2.0 devices that do not support bulk streams. When we're working
with a non-streams device, we need to be able to uniquely identify a
SCSI command with a tag in the IU. Devices will barf and abort all
queued commands if they find a duplicate tag.
uas_queuecommand_lck() sets cmdinfo->stream to zero if the device
doesn't support streams, which is later passed into uas_alloc_cmd_urb()
as the variable stream. This means the UAS driver was setting the tag
in all commands to zero for non-stream devices. So the UAS driver won't
currently work with USB 2.0 devices.
Use the SCSI command tag instead of the stream ID for the command IU
tag. We have to add one to the SCSI command tag because SCSI tags are
zero-based, but stream IDs are one-based, and the command tag must match
the stream ID that we're queueing the data IUs for. Untagged SCSI
commands use stream ID 1.

If the original submission (or allocation) of the URBs for a SCSI
command fails, the UAS driver sticks the command structure in a
workqueue and schedules uas_do_work() to run. That function removes the
entire queue before walking across it and attempting to resubmit.
Unfortunately, if the second submission fails, we will leak memory
(because an allocated URB was not submitted) and possibly leave the SCSI
command partially enqueued on some of the stream rings. Fix this by
checking whether the second submission failed and re-queueing the
command to the UAS workqueue and scheduling it.

module_usb_driver() macro which makes the code smaller and a bit
simpler.
Added bonus is that it removes some unneeded kernel log messages about

Move the mid-layer's ->queuecommand() invocation from being locked
with the host lock to being unlocked to facilitate speeding up the
critical path for drivers who don't need this lock taken anyway.
The patch below presents a simple SCSI host lock push-down as an
equivalent transformation. No locking or other behavior should change
with this patch. All existing bugs and locking orders are preserved.
Additionally, add one parameter to queuecommand,
struct Scsi_Host *
and remove one parameter from queuecommand,
void (*done)(struct scsi_cmnd *)
Scsi_Host* is a convenient pointer that most host drivers need anyway,
and 'done' is redundant to struct scsi_cmnd->scsi_done.
Minimal code disturbance was attempted with this change. Most drivers
needed only two one-line modifications for their host lock push-down.

REQ_HARDBARRIER is dead now, so remove the leftovers. What's left
at this point is:
- various checks inside the block layer.
- sanity checks in bio based drivers.
- now unused bio_empty_barrier helper.
- Xen blockfront use of BLKIF_OP_WRITE_BARRIER - it's dead for a while,
but Xen really needs to sort out it's barrier situaton.
- setting of ordered tags in uas - dead code copied from old scsi
drivers.
- scsi different retry for barriers - it's dead and should have been
removed when flushes were converted to FS requests.
- blktrace handling of barriers - removed. Someone who knows blktrace
better should add support for REQ_FLUSH and REQ_FUA, though.
