Pull clk framework updates from Mike Turquette:
"This is much later than usual due to several last minute bugs that had
to be addressed. As usual the majority of changes are new drivers and
modifications to existing drivers. The core recieved many fixes along
with the groundwork for several large changes coming in the future
which will better parition clock providers from clock consumers"

"After stopping the full x86/apic branch, I took some time to go
through the first block of patches again, which are mostly cleanups
and preparatory work for the irqdomain conversion and ioapic hotplug
support.
Unfortunaly one of the real problematic commits was right at the
beginning, so I rebased this portion of the pending patches without
the offenders.
It would be great to get this into 3.19. That makes reworking the
problematic parts simpler. The usual tip testing did not unearth any
issues and it is fully bisectible now.
I'm pretty confident that this wont affect the calmness of the xmas
season.
Changes:
- Split the convoluted io_apic.c code into domain specific parts
(vector, ioapic, msi, htirq)
- Introduce proper helper functions to retrieve irq specific data
instead of open coded dereferencing of pointers
- Preparatory work for ioapic hotplug and irqdomain conversion

- Removal of unused irq entry stubs
- Make native_smp_prepare_cpus() preemtible to avoid GFP_ATOMIC

Having switched over all of the users of CONFIG_PM_RUNTIME to use
CONFIG_PM directly, turn the latter into a user-selectable option
and drop the former entirely from the tree.

I'm leaving Red Hat at the end of December 2014, so remove all

The new driver is around for more than 2 years now, so the old one can
go. Getting rid of it helps the removal of the legacy .attach_adapter
callback of the I2C subsystem.

The 8250_omap serial driver is a new user of CONFIG_PM_RUNTIME.
However, after commit b2b49ccbdd54 (PM: Kconfig: Set PM_RUNTIME if
PM_SLEEP is selected) PM_RUNTIME is always set if PM is set, so
#ifdef blocks depending on CONFIG_PM_RUNTIME may now be changed to
depend on CONFIG_PM.
Replace CONFIG_PM_RUNTIME with CONFIG_PM in 8250_omap.c.

A couple of new CONFIG_PM_RUNTIME users have been added recently
in the SPI subsystem.
However, after commit b2b49ccbdd54 (PM: Kconfig: Set PM_RUNTIME if
PM_SLEEP is selected) PM_RUNTIME is always set if PM is set, so
#ifdef blocks depending on CONFIG_PM_RUNTIME may now be changed to
depend on CONFIG_PM.
Replace CONFIG_PM_RUNTIME with CONFIG_PM everywhere under
drivers/spi/ (again).

commit 4dbd27711cd9 "tick: export nohz tick idle symbols for module
use" was merged via the thermal tree without an explicit ack from the
relevant maintainers.
The exports are abused by the intel powerclamp driver which implements
a fake idle state from a sched FIFO task. This causes all kinds of
wreckage in the NOHZ core code which rightfully assumes that
tick_nohz_idle_enter/exit() are only called from the idle task itself.
Recent changes in the NOHZ core lead to a failure of the powerclamp
driver and now people try to hack completely broken and backwards
workarounds into the NOHZ core code. This is completely unacceptable
and just papers over the real problem. There are way more subtle
issues lurking around the corner.
The real solution is to fix the powerclamp driver by rewriting it with
a sane concept, but that's beyond the scope of this.
So the only solution for now is to remove the calls into the core NOHZ
code from the powerclamp trainwreck along with the exports.

Further, the big virtio 1.0 conversion missed a couple of places.

This isn't 100% sparse-clean yet because on many architectures

I posted a patchset to fix it up by adding __force on all arches that

"These are regression fixes (leds-gpio, ACPI backlight driver,
operating performance points library, ACPI device enumeration
messages, cpupower tool), other bug fixes (ACPI EC driver, ACPI device
PM), some cleanups in the operating performance points (OPP)
framework, continuation of CONFIG_PM_RUNTIME elimination, a couple of
minor intel_pstate driver changes, a new MAINTAINERS entry for it and
an ACPI fan driver change needed for better support of thermal
management in user space.
Specifics:
- Fix a regression in leds-gpio introduced by a recent commit that
inadvertently changed the name of one of the properties used by the
driver (Fabio Estevam).
- Fix a regression in the ACPI backlight driver introduced by a
recent fix that missed one special case that had to be taken into
account (Aaron Lu).
- Drop the level of some new kernel messages from the ACPI core
introduced by a recent commit to KERN_DEBUG which they should have
used from the start and drop some other unuseful KERN_ERR messages
printed by ACPI (Rafael J Wysocki).
- Revert an incorrect commit modifying the cpupower tool (Prarit
Bhargava).
- Fix two regressions introduced by recent commits in the OPP library
and clean up some existing minor issues in that code (Viresh
Kumar).
- Continue to replace CONFIG_PM_RUNTIME with CONFIG_PM throughout the
tree (or drop it where that can be done) in order to make it
possible to eliminate CONFIG_PM_RUNTIME (Rafael J Wysocki, Ulf
Hansson, Ludovic Desroches).
There will be one more "CONFIG_PM_RUNTIME removal" batch after this
one, because some new uses of it have been introduced during the
current merge window, but that should be sufficient to finally get
rid of it.
- Make the ACPI EC driver more robust against race conditions related
to GPE handler installation failures (Lv Zheng).
- Prevent the ACPI device PM core code from attempting to disable
GPEs that it has not enabled which confuses ACPICA and makes it
report errors unnecessarily (Rafael J Wysocki).
- Add a "force" command line switch to the intel_pstate driver to
make it possible to override the blacklisting of some systems in
that driver if needed (Ethan Zhao).
- Improve intel_pstate code documentation and add a MAINTAINERS entry
for it (Kristen Carlson Accardi).
- Make the ACPI fan driver create cooling device interfaces witn
names that reflect the IDs of the ACPI device objects they are
associated with, except for "generic" ACPI fans (PNP ID "PNP0C0B").
That's necessary for user space thermal management tools to be able
to connect the fans with the parts of the system they are supposed

This patch changes core_scsi3_pro_release() logic to allow an
existing AllRegistrants type reservation to be re-reserved by
any registered I_T nexus.
This addresses a issue where AllRegistrants type RESERVE was
receiving RESERVATION_CONFLICT status if dev_pr_res_holder did
not match the same I_T nexus, instead of just returning GOOD
status following spc4r34 Section 5.9.9:
"If the device server receives a PERSISTENT RESERVE OUT command
with RESERVE service action where the TYPE field and the SCOPE
field contain the same values as the existing type and scope
from a persistent reservation holder, it shall not make any
change to the existing persistent reservation and shall complete
the command with GOOD status."

An earlier commit to resolve an issue with encapsulation offloads missed

Commit bc96f648df1bbc2729abbb84513cf4f64273a1f1 (xen-netback: make
feature-rx-notify mandatory) incorrectly assumed that there were no
frontends in use that did not support this feature. But the frontend
driver in MiniOS does not and since this is used by (qemu) stubdoms,
these stopped working.
Netback sort of works as-is in this mode except:
- If there are no Rx requests and the internal Rx queue fills, only
the drain timeout will wake the thread. The default drain timeout
of 10 s would give unacceptable pauses.
- If an Rx stall was detected and the internal Rx queue is drained,
then the Rx thread would never wake.
Handle these two cases (when feature-rx-notify is disabled) by:
- Reducing the drain timeout to 30 ms.
- Disabling Rx stall detection.

DMA is opt-in for this driver. So, we can't use deferred probing for
requesting DMA channels in probe, because our driver would get endlessly
deferred if DMA support is compiled in AND the DMA driver is missing.
Because we can't know when the DMA driver might show up, we always try
again when a DMA transfer would be possible. The downside is that there
is more overhead for setting up PIO transfers under the above scenario.
But well, having DMA enabled and the proper DMA driver missing looks
like a broken or test config anyhow.

Originally, the I2C controller supported by the i2c-mv64xxx driver
requires a lot of software support: an interrupt is generated at each
step of an I2C transaction (after the start bit, after sending the
address, etc.) and the driver is in charge of re-programming the I2C
controller to do the next step of the I2C transaction. This explains
the fairly complex state machine that the driver has.
On Marvell Armada XP and later processors (Armada 375, 38x, etc.), the
I2C controller was extended with a part called the "I2C Bridge", which
allows to offload the I2C transaction completely to the
hardware. Initial support for this mechanism was added in commit
930ab3d403a ("i2c: mv64xxx: Add I2C Transaction Generator support").
However, the implementation done in this commit has two related
issues, which this commit fixes by completely changing how the offload
implementation is done:
* SMBus read transfers, where there is one write to select the
register immediately followed in the same transaction by one read,
were making the processor hang. This was easier visible on the
Marvell Armada XP WRT1900AC platform using a driver for an I2C LED
controller, or on other Armada XP platforms by using a simple
'i2cget' command to read an I2C EEPROM.
* The implementation was based on the fact that the offload engine
was re-programmed to transfer each message of an I2C xfer: this
meant that each message sent with the offload engine was starting
with a normal I2C start sequence. However, the I2C subsystem
assumes that all messages belonging to the same xfer will use the
so-called "repeated start" so that the entire I2C xfer is seen as
one transfer by the I2C devices and cannot be interrupt by other
I2C masters on the same bus.
In fact, the "I2C Bridge" allows to offload three types of xfer:
- xfer of one write message
- xfer of one read message
- xfer of one write message followed by one read message
For all other situations, we have to fallback to not using the "I2C
Bridge" in order to get proper I2C semantics.
Therefore, this commit reworks the offload implementation to put it
not at the message level, but at the xfer level: in the
mv64xxx_i2c_xfer() function, we decide if the transaction can be
offloaded (in which case it is handled by the
mv64xxx_i2c_offload_xfer() function), or otherwise it is handled by
the slow path (implemented in the existing mv64xxx_i2c_execute_msg()).
This allows to simplify the state machine, which no longer needs to
have any state related to the offload implementation: the offload
implementation is now completely separated from the slow path (with
the exception of the interrupt handler, of course).
In summary:
- mv64xxx_i2c_can_offload() will analyze an I2C xfer and decided of
the "I2C Bridge" can be used to offload it or not.
- mv64xxx_i2c_offload_xfer() will actually program the "I2C Bridge"
to offload one xfer (of either one or two messages), and block
using mv64xxx_i2c_wait_for_completion() until the xfer completes.
- The interrupt handler mv64xxx_i2c_intr() is modified to push the
offload related code to a separate function,
mv64xxx_i2c_intr_offload(). It will take care of reading the
received data if needed.
This commit was tested on:
- Armada XP OpenBlocks AX3-4 (EEPROM on I2C and RTC on I2C)
- Armada XP WRT1900AC (LED controller on I2C)
- Armada XP GP (EEPROM on I2C)

CEPH_OSD_OP_DELETE is not an extent op, stop treating it as such. This
sneaked in with discard patches - it's one of the three osd ops (the
other two are CEPH_OSD_OP_TRUNCATE and CEPH_OSD_OP_ZERO) that discard
is implemented with.

The functions ceph_put_snap_context() and iput() test whether their
argument is NULL and then return immediately. Thus the test around the
call is not needed.
This issue was detected by using the Coccinelle software.

domain being gated
Audio subsystem clocks are located in separate block. On Exynos 5420 if
clock for this block (from main clock domain) 'mau_epll' is gated then
any read or write to audss registers will block.
This kind of boot hang was observed on Arndale Octa and Peach Pi/Pit
after introducing runtime PM to pl330 DMA driver. After that commit the
'mau_epll' was gated, because the "amba" clock was disabled and there
were no more users of mau_epll.
The system hang on one of steps:
1. Disabling unused clocks from audss block.
2. During audss GPIO setup (just before probing i2s0 because
samsung_pinmux_setup() tried to access memory from audss block which was
gated.
Add a workaround for this by enabling the 'mau_epll' clock in probe.

ancient and hopelessly obsolete. These drivers also need to be converted
to newer media frameworks but due to the lack of hardware that's going
to be impossible. In addition, cheaper and vastly better hardware is
available today.
So these drivers are a prime candidate for removal. If someone is
interested in working on these drivers to prevent their removal, then
please contact the linux-media mailinglist.
Let's be honest, the age of parallel port webcams and ISA video capture
boards is really gone.

This driver hasn't been tested in a long, long time. The company that made
this chip has gone bust many years ago and hardware using this chip is next
to impossible to find.
This driver needs to be converted to newer media frameworks but due to the
lack of hardware that's going to be impossible. Since cheap alternatives are
easily available, there is little point in keeping this driver alive.
In other words, this driver is a prime candidate for removal. If someone is
interested in working on this driver to prevent its removal, then please
contact the linux-media mailinglist.

The start_streaming op is responsible for starting the video dma,
so it shouldn't be called anymore from the buf_queue op.
Unfortunately, this call to start_video_dma() was added to the
start_streaming op, but was forgotten to be removed from the
buf_queue op, which is where it used to be before the vb2 conversion.
Calling this function twice causes very hard to find errors: sometimes
it works, sometimes it doesn't. It took me a whole friggin' day
to track this down, and in the end it was just luck that my eye suddenly
triggered on that line.

The cx88 vb2 conversion and the vb2 dma_sg improvements were developed separately and
were merged separately. Unfortunately, the patch updating drivers to the dma_sg
improvements didn't take the updated cx88 driver into account. Basically two ships
passing in the night, unaware of one another even though both ships have the same
owner, i.e. me :-)

The current implementations all use dev_uc_add_excl() and such whose API
doesn't support vlans, so we can't make it with NICs HW for now.

The Exynos IOMMU driver uses the ARM specific dmac_flush_range() and
outer_flush_range() functions. This breaks the build on arm64 allmodconfig
in -next since support has been merged for some Exynos ARMv8 SoCs. Add a
dependency on ARM to keep things building until either the driver has the
ARM dependencies removed or the ARMv8 architecture code implements these
ARM specific APIs.

This commit contains 2 fixes for the 128B CQE/EQE stride feaure.
Wei found that mlx4_QUERY_HCA function marked the wrong capability
in flags (64B CQE/EQE), when CQE/EQE stride feature was enabled.
Also added small fix in initial CQE ownership bit assignment, when CQE
is size is not default 32B.

After d75b1ade567ffab085e8adbbdacf0092d10cd09c (net: less interrupt
masking in NAPI) the napi instance is removed from the per-cpu list
prior to calling the n->poll(), and is only requeued if all of the
budget was used. This inadvertently broke netfront because netfront
does not use NAPI correctly.
If netfront had not used all of its budget it would do a final check
for any Rx responses and avoid calling napi_complete() if there were
more responses. It would still return under budget so it would never
be rescheduled. The final check would also not re-enable the Rx
interrupt.
Additionally, xenvif_poll() would also call napi_complete() /after/
enabling the interrupt. This resulted in a race between the
napi_complete() and the napi_schedule() in the interrupt handler. The
use of local_irq_save/restore() avoided by race iff the handler is
running on the same CPU but not if it was running on a different CPU.
Fix both of these by always calling napi_compete() if the budget was
not all used, and then calling napi_schedule() if the final checks
says there's more work.

Otherwise we get things like:

flags field in ifreq is only 16 bit wide, but
we read it as a 32 bit value.
If userspace doesn't zero-initialize unused fields,
this will lead to failures.

Now we have splitted functions to support MSI and HT_IRQ into vector.c,
and they have no dependency on IOAPIC any more. So change Kconfig files
to make MSI and HT_IRQ independent of X86_IO_APIC.

None of the callers requires irq_attr to be filled
in. IO_APIC_get_PCI_irq_vector() does not do anything useful with it
either.

To keep balance of IOAPIC pin reference count, we need to protect
pirq_enable_irq(), acpi_pci_irq_enable() and intel_mid_pci_irq_enable()
from reentrance. There are two cases which will cause reentrance.
The first case is caused by suspend/hibernation. If pcibios_disable_irq
is called during suspending/hibernating, we don't release the assigned
IRQ number, otherwise it may break the suspend/hibernation. So late when
pcibios_enable_irq is called during resume, we shouldn't allocate IRQ
number again.
The second case is that function acpi_pci_irq_enable() may be called
twice for PCI devices present at boot time as below:
1) pci_acpi_init()
--> acpi_pci_irq_enable() if pci_routeirq is true
2) pci_enable_device()
--> pcibios_enable_device()
--> acpi_pci_irq_enable()
We can't kill kernel parameter pci_routeirq yet because it's still
needed for debugging purpose.
So flag irq_managed is introduced to track whether IRQ number is
assigned by OS and to protect pirq_enable_irq(), acpi_pci_irq_enable()
and intel_mid_pci_irq_enable() from reentrance.

To support IOAPIC hotplug on x86 and IA64 platforms, OS needs to figure
out global interrupt source number(GSI) and IOAPIC enumeration ID
through ACPI interfaces. So BIOS must implement an ACPI IOAPIC device
with _GSB/_UID or _MAT method to support IOAPIC hotplug. OS also needs
to figure out base physical address to access IOAPIC registers. OS may
get the base physical address through PCI BARs if IOAPIC device is
visible in PCI domain, otherwise OS may get the address by ACPI _CRS
method if IOAPIC device is hidden from PCI domain by BIOS.
When adding a PCI subtree, we need to add IOAPIC devices before enabling
all other PCI devices because other PCI devices may use the IOAPIC to
allocate PCI interrupts.
So we plan to reimplement IOAPIC driver as an ACPI instead of PCI driver
due to:
1) hot-pluggable IOAPIC devices are always visible in ACPI domain,
but may or may not be visible in PCI domain.
2) we could explicitly control the order between IOAPIC and other PCI
devices.
We also have another choice to use a PCI driver to manage IOAPIC device
if it's visible in PCI domain and use an ACPI driver if it's only
visible in ACPI domain. But this solution is a little complex.
It shouldn't cause serious backward compatibility issues because:
1) IOAPIC hotplug is never supported on x86 yet because it hasn't
implemented the required acpi_register_ioapic() and
acpi_unregister_ioapic().
2) Currently only ACPI based IOAPIC hotplug is possible on x86 and
IA64, we don't know other specifications and interfaces to support
IOAPIC hotplug yet.
3) We will reimplement an ACPI IOAPIC driver to support IOAPIC hotplug.
This change also helps to get rid of the false alarm on all current
Linux distributions:

This patch changes iscsit_do_tx_data() to fail on short writes
when kernel_sendmsg() returns a value different than requested
transfer length, returning -EPIPE and thus causing a connection
reset to occur.
This avoids a potential bug in the original code where a short
write would result in kernel_sendmsg() being called again with
the original iovec base + length.
In practice this has not been an issue because iscsit_do_tx_data()
is only used for transferring 48 byte headers + 4 byte digests,
along with seldom used control payloads from NOPIN + TEXT_RSP +
REJECT with less than 32k of data.
So following Al's audit of iovec consumers, go ahead and fail
the connection on short writes for now, and remove the bogus
logic ahead of his proper upstream fix.

This patch fixes an issue with AllRegistrants reservations where
an unregister operation by the I_T nexus reservation holder would
incorrectly drop the reservation, instead of waiting until the
last active I_T nexus is unregistered as per SPC-4.
This includes updating __core_scsi3_complete_pro_release() to reset
dev->dev_pr_res_holder with another pr_reg for this special case,
as well as a new 'unreg' parameter to determine when the release
is occuring from an implicit unregister, vs. explicit RELEASE.
It also adds special handling in core_scsi3_free_pr_reg_from_nacl()
to release the left-over pr_res_holder, now that pr_reg is deleted
from pr_reg_list within __core_scsi3_complete_pro_release().

This patch fixes the usage of R_HOLDER bit for an All Registrants
reservation in READ_FULL_STATUS, where only the registration who
issued RESERVE was being reported as having an active reservation.
It changes core_scsi3_pri_read_full_status() to check ahead of the
list walk of active registrations to see if All Registrants is active,
and if so set R_HOLDER bit and scope/type fields for all active
registrations.

This patch implement a page fault handler (leaving the pages pinned as
of time being). The page fault handler handles initiator and responder

The new function allows updating the page tables of a memory region
after it was created. This can be used to handle page faults and page
invalidations.
Since mlx5_ib_update_mtt will need to work from within page invalidation,
so it must not block on memory allocation. It employs an atomic memory
allocation mechanism that is used as a fallback when kmalloc(GFP_ATOMIC) fails.
In order to reuse code from mlx5_ib_populate_pas, the patch splits
this function and add the needed parameters.

This patch wraps together several changes needed for on-demand paging support
in the mlx5_ib_populate_pas function, and when registering memory regions.
* Instead of accepting a UMR bit telling the function to enable all
access flags, the function now accepts the access flags themselves.
* For on-demand paging memory regions, fill the memory tables from the
correct list, and enable/disable the access flags per-page according
to whether the page is present.
* A new bit is set to enable writing of access flags when using the
firmware create_mkey command.
* Disable contig pages when on-demand paging is enabled.
In addition the patch changes the UMR code to use PTR_ALIGN instead of
our own macro.

The patch adds infrastructure to query ODP capabilities in the mlx5
driver. The code will read the capabilities from the device, and
enable only those capabilities that both the driver and the device
supports. At this point ODP is not supported, so no capability is
copied from the device, but the patch exposes the global ODP device
capability bit.

* Add an interval tree implementation for ODP umems. Create an
interval tree for each ucontext (including a count of the number of
ODP MRs in this context, semaphore, etc.), and register ODP umems in
the interval tree.
* Add MMU notifiers handling functions, using the interval tree to
notify only the relevant umems and underlying MRs.
* Register to receive MMU notifier events from the MM subsystem upon
ODP MR registration (and unregister accordingly).
* Add a completion object to synchronize the destruction of ODP umems.
* Add mechanism to abort page faults when there's a concurrent invalidation.
The way we synchronize between concurrent invalidations and page
faults is by keeping a counter of currently running invalidations, and
a sequence number that is incremented whenever an invalidation is
caught. The page fault code checks the counter and also verifies that
the sequence number hasn't progressed before it updates the umem's
page tables. This is similar to what the kvm module does.
In order to prevent the case where we register a umem in the middle of
an ongoing notifier, we also keep a per ucontext counter of the total
number of active mmu notifiers. We only enable new umems when all the
running notifiers complete.

* Extend the umem struct to keep the ODP related data.
* Allocate and initialize the ODP related information in the umem
(page_list, dma_list) and freeing as needed in the end of the run.
* Store a reference to the process PID struct in the ucontext. Used to
safely obtain the task_struct and the mm during fault handling,
without preventing the task destruction if needed.
* Add 2 helper functions: ib_umem_odp_map_dma_pages and
ib_umem_odp_unmap_dma_pages. These functions get the DMA addresses
of specific pages of the umem (and, currently, pin them).
* Support for page faults only - IB core will keep the reference on
the pages used and call put_page when freeing an ODP umem
area. Invalidations support will be added in a later patch.

the infiniband subsystem (CONFIG_INFINIBAND_ON_DEMAND_PAGING). In a
later patch, this configuration option will select the MMU_NOTIFIER
configuration option to enable mmu notifiers.
* Add a flag for on demand paging (ODP) support in the IB device capabilities.
* Add a flag to request ODP MR in the access flags to reg_mr.
* Fail registrations done with the ODP flag when the low-level driver
doesn't support this.
* Change the conditions in which an MR will be writable to explicitly
specify the access flags. This is to avoid making an MR writable just
because it is an ODP MR.
* Add a ODP capabilities to the extended query device verb.

Add extensible query device capabilities verb to allow adding new features.
ib_uverbs_ex_query_device is added and copy_query_dev_fields is used to
copy capability fields to be used by both ib_uverbs_query_device and

In some drivers there's a need to read data from a user space area
that was pinned using ib_umem when running from a different process
context.
The ib_umem_copy_from function allows reading data from the physical
pages pinned in the ib_umem struct.

In order to allow umems that do not pin memory, we need the umem to
keep track of its region's address.
This makes the offset field redundant, and so this patch removes it.

The current UMR interface doesn't allow partial updates to a memory
region's page tables. This patch changes the interface to allow that.
It also changes the way the UMR operation validates the memory
region's state. When set, IB_SEND_UMR_FAIL_IF_FREE will cause the UMR
operation to fail if the MKEY is in the free state. When it is
unchecked the operation will check that it isn't in the free state.

Since UMR code now uses its own context struct on the stack, the pas
and dma pointers for the UMR operation that remained in the mlx5_ib_mr
struct are not necessary. This patch removes them.

When closing the connection, we should first terminate the connection
(in case it was not previously terminated) to guarantee the QP is in
error state and we are done with servicing IO. Only then go ahead with
tasks cleanup via iscsi_conn_stop.

In certain scenarios (target kill with live IO) scsi TMFs may race
with iser RDMA teardown, which might cause NULL dereference on iser IB
device handle (which might have been freed). In this case we take a
conditional lock for TMFs and check the connection state (avoid
introducing lock contention in the IO path). This is indeed best
effort approach, but sufficient to survive multi targets sudden death
while heavy IO is inflight.
While we are on it, add a nice kernel-doc style documentation.

Fix a regression was introduced in commit 6df5a128f0fd ("IB/iser:
Suppress scsi command send completions").
The sig_count was wrongly set to be static variable, thus it is
possible that we won't reach to (sig_count % ISER_SIGNAL_BATCH) == 0
condition (due to races) and the send queue will be overflowed.
Instead keep sig_count per connection. We don't need it to be atomic
as we are safe under the iscsi session frwd_lock taken by libiscsi on
the queuecommand path.

When creating a connection QP we choose the least used CQ and inc the
number of active QPs on that. If we fail to create the QP, we need to
decrement the active QPs counter.

No real need to wait for TIMEWAIT_EXIT before we destroy the RDMA
resources (also TIMEAWAIT_EXIT is not guarenteed to always arrive). As
for the cma_id, only destroy it if the state is not DOWN where in this
case, conn_release is already running and we don't want to compete.

In case of the HCA going into catasrophic error flow, the
beacon post_send is likely to fail, so surely there will
be no completion for it.
In this case, use a best effort approach and don't wait for beacon
completion if we failed to post the send.

Various places in the IPoIB code had a deadlock related to flushing
the ipoib workqueue. Now that we have per device workqueues and a
specific flush workqueue, there is no longer a deadlock issue with
flushing the device specific workqueues and we can do so unilaterally.

We used to pass a flush variable to mcast_stop_thread to indicate if
we should flush the workqueue or not. This was due to some code
trying to flush a workqueue that it was currently running on which is
a no-no. Now that we have per-device work queues, and now that
ipoib_mcast_restart_task has taken the fact that it is queued on a
single thread workqueue with all of the ipoib_mcast_join_task's and
therefore has no need to stop the join task while it runs, we can do
away with the flush parameter and unilaterally flush always.

During my recent work on the rtnl lock deadlock in the IPoIB driver, I
saw that even once I fixed the apparent races for a single device, as
soon as that device had any children, new races popped up. It turns
out that this is because no matter how well we protect against races
on a single device, the fact that all devices use the same workqueue,
and flush_workqueue() flushes *everything* from that workqueue, we can
have one device in the middle of a down and holding the rtnl lock and
another totally unrelated device needing to run mcast_restart_task,
which wants the rtnl lock and will loop trying to take it unless is
sees its own FLAG_ADMIN_UP flag go away. Because the unrelated
interface will never see its own ADMIN_UP flag drop, the interface
going down will deadlock trying to flush the queue. There are several
possible solutions to this problem:
Make carrier_on_task and mcast_restart_task try to take the rtnl for
some set period of time and if they fail, then bail. This runs the
real risk of dropping work on the floor, which can end up being its
own separate kind of deadlock.
Set some global flag in the driver that says some device is in the
middle of going down, letting all tasks know to bail. Again, this can
drop work on the floor. I suppose if our own ADMIN_UP flag doesn't go
away, then maybe after a few tries on the rtnl lock we can queue our
own task back up as a delayed work and return and avoid dropping work
on the floor that way. But I'm not 100% convinced that we won't cause
other problems.
Or the method this patch attempts to use, which is when we bring an
interface up, create a workqueue specifically for that interface, so
that when we take it back down, we are flushing only those tasks
associated with our interface. In addition, keep the global
workqueue, but now limit it to only flush tasks. In this way, the
flush tasks can always flush the device specific work queues without
having deadlock issues.

In preparation for using per device work queues, we need to move the
start of the neighbor thread task to after ipoib_ib_dev_init and move
the destruction of the neighbor task to before ipoib_ib_dev_cleanup.
Otherwise we will end up freeing our workqueue with work possibly
still on it.

Our mcast_dev_flush routine and our mcast_restart_task can race
against each other. In particular, they both hold the priv->lock
while manipulating the rbtree and while removing mcast entries from
the multicast_list and while adding entries to the remove_list, but
they also both drop their locks prior to doing the actual removes.
The mcast_dev_flush routine is run entirely under the rtnl lock and so
has at least some locking. The actual race condition is like this:

core/multicast.c is ref counted, we run into an unbalanced refcount
issue. To avoid stomping on each others removes, take the rtnl lock
specifically when we are deleting the entries from the remove list.

Commit a9c8ba5884 ("IPoIB: Fix usage of uninitialized multicast
objects") added a new flag MCAST_JOIN_STARTED, but was not very strict
in how it was used. We didn't always initialize the completion struct
before we set the flag, and we didn't always call complete on the
completion struct from all paths that complete it. This made it less
than totally effective, and certainly made its use confusing. And in
the flush function we would use the presence of this flag to signal
that we should wait on the completion struct, but we never cleared
this flag, ever. This is further muddied by the fact that we overload
the MCAST_FLAG_BUSY flag to mean two different things: we have a join
in flight, and we have succeeded in getting an ib_sa_join_multicast.
In order to make things clearer and aid in resolving the rtnl deadlock
bug I've been chasing, I cleaned this up a bit.
1) Remove the MCAST_JOIN_STARTED flag entirely
2) Un-overload MCAST_FLAG_BUSY so it now only means a join is in-flight
3) Test on mcast->mc directly to see if we have completed
ib_sa_join_multicast (using IS_ERR_OR_NULL)
4) Make sure that before setting MCAST_FLAG_BUSY we always initialize
the mcast->done completion struct
5) Make sure that before calling complete(&mcast->done), we always clear
the MCAST_FLAG_BUSY bit
6) Take the mcast_mutex before we call ib_sa_multicast_join and also
take the mutex in our join callback. This forces
ib_sa_multicast_join to return and set mcast->mc before we process
the callback. This way, our callback can safely clear mcast->mc
if there is an error on the join and we will do the right thing as
a result in mcast_dev_flush.
7) Because we need the mutex to synchronize mcast->mc, we can no
longer call mcast_sendonly_join directly from mcast_send and
instead must add sendonly join processing to the mcast_join_task
A number of different races are resolved with these changes. These
races existed with the old MCAST_FLAG_BUSY usage, the
MCAST_JOIN_STARTED flag was an attempt to address them, and while it
helped, a determined effort could still trip things up.

By holding the mutex around ib_sa_multicast_join and taking the mutex
early in the callback, we force mcast->mc to be valid at the time we
run the callback. This allows us to clear mcast->mc if there is an
error and the join is going to fail. We do this before we complete
the mcast. In this way, mcast_dev_flush always sees consistent state
in regards to mcast->mc membership at the time that the
wait_for_completion() returns.

We blindly assume that we can just take the rtnl lock and that will
prevent races with downing this interface. Unfortunately, that's not
the case. In ipoib_mcast_stop_thread() we will call flush_workqueue()
in an attempt to clear out all remaining instances of ipoib_join_task.
But, since this task is put on the same workqueue as the join task,
the flush_workqueue waits on this thread too. But this thread is
deadlocked on the rtnl lock. The better thing here is to use trylock
and loop on that until we either get the lock or we see that
FLAG_ADMIN_UP has been cleared, in which case we don't need to do
anything anyway and we just return.

When transitioning into ERROR state, the QP was getting flushed after
waking up any waiters. This can cause applications to miss flushed work

0B MRs need some tweaks to work correctly with HW. When writing the
TPTE, if the MR length is zero we now:
1) turn off all permissions
2) set the length to -1
While functionality/capabilities of the MR are the same with these
changes, it resolves a dapltest 0B RDMA Read test failure. Based on

Address resolution always does a context switch to a work-queue to
deliver the address resolution event. When the IP address is already
cached in the system ARP table, we're going through the following:
chain:
rdma_resolve_ip --> addr_resolve (cache hit) -->
which ends up with:
queue_req --> set_timeout (now) --> mod_delayed_work(,, delay=1)
We actually do realize that the timeout should be zero, but the code
forces it to a minimum of one jiffie.
Using one jiffie as the minimum delay value results in sub-optimal
scheduling of executing this work item by the workqueue, which on the
below testbed costs about 3-4ms out of 12ms total time.
To fix that, we let the minimum delay to be zero. Note that the
connect step times change too, as there are address resolution calls
from that flow.
The results were taken from running both client and server on the
same node, over mlx4 RoCE port.

Applications can request that the SM assign an MGID by passing a mcast
member request containing MGID = 0. When the SM responds by sending
the allocated MGID, this MGID replaces the 0-MGID in the multicast group.
However, the MGID field in the group is also the key field in the IB
core multicast code rbtree containing the multicast groups for the
port.
Since this is a key field, correct handling requires that the group
entry be deleted from the rbtree and then re-inserted with the new
key, so that the table structure is properly maintained.
The current code does not do this correctly. Correct operation
requires that if the key-field gid has changed at all, it should be
deleted and re-inserted.
Note that when inserting, if the new MGID is zero (not the case here
but the code should handle this correctly), we allow duplicate entries
for 0-MGIDs.

For RoCE, resolution of layer 2 address attributes forces no VLAN if
link-local GIDs are used. This patch allows applications to choose
the VLAN ID for link-local based RoCE GIDs by setting IB_QP_VID in
their QP attribute mask, and prevents the core from overriding this
choice.

"Here's the big staging tree pull request for 3.19-rc1.
We continued to delete more lines than were added, always a good
thing, but not at a huge rate this release, only about 70k lines
removed overall mostly from removing the horrid bcm driver.
Lots of normal staging driver cleanups and fixes all over the place,
well over a thousand of them, the shortlog shows all the horrid
details.
The "contentious" thing here is the movement of the Android binder
code out of staging into the "real" part of the kernel. This is code
that has been stable for a few years now and is working as-is in the
tens of millions of devices with no issues. Yes, the code is horrid,
and the userspace api leaves a lot to be desired, but it's not going
to change due to legacy issues that we have no control over. Because
so many devices and companies rely on this, and the code is stable,
might as well promote it out of staging.
This was all discussed at the Linux Plumbers conference, and everyone
participating agreed that this was the best way forward.
There is work happening to replace the binder code with something new
that is happening right now, but I don't expect to see the results of
that work for another year at the earliest. If that ever happens, and
Android switches over to it, I'll gladly remove this version.
As for maintainers, I'll be glad to maintain this code, I've been
doing it for the past few years with no problems. I'll send a
MAINTAINERS entry for it before 3.19-final is out, still need to talk
to the Google developers about if they are willing to help with it or
not, last I checked they were, which was good.
All of these patches have been in linux-next for a while with no
reported issues"

Most of the patches are clock-related. The DPLL implementation is
changed to better align to the common clock framework.

Commit 6314b6796e3c (clk: Don't hold prepare_lock across debugfs
creation, 2014-09-04) forgot to update one place where we hold
the prepare_lock while creating debugfs directories. This means
we still have the chance of a deadlock that the commit was trying
to fix. Actually fix it by moving the debugfs creation outside
the prepare_lock.

Debug code prints the fifo name via custom dev_warn() wrappers. The
fifo_names array is only non-zero when debugging is manually enabled,
which is all well and good. However, it's *not* good that this array
uses zero-length arrays in the non-debug case, and so it doesn't
actually have any memory allocated to it. This means that as far as we
know, fifo_names[i] actually points to garbage memory.
I've seen this in my log:

So let's give this array space enough to fill it with a NULL byte.

Inside macb_probe(), when devm_request_irq() fails on queue q, there is no need
to call devm_free_irq() on queues 0..q-1 because the managed device resources
are released later when calling free_netdev().
Also removing devm_free_irq() call from macb_remove() for the same reason.

To support asymmetric EQ allocations, we should query the device
capabilities prior to enabling SRIOV. As a side effect of adding that,
we are dumping the PF device capabilities twice. Avoid that by moving
the printing into a helper function which is called once.

The current mlx4_load_one has a memory leak as it always allocates
dev_cap, but frees it only on error.
In addition, even if VFs exist when mlx4_load_one is called,
we still need to notify probed VFs that we're loading (by
incrementing pf_loading).

If the firmware has declared more than 8 video output devices, and the
one that control the internal panel's backlight is listed after the
first 8 output devices, the _DOD will not include it due to the current
i915 operation region implementation. As a result, we will not create a
backlight device for it while we should. Solve this problem by special
case the firmware that has 8+ output devices in that if we see such a
firmware, we do not test if the device is in _DOD list. The creation of
the backlight device will also enable the firmware to emit events on
backlight hotkey press when the acpi_osi= cmdline option is specified on
those affected ASUS laptops.

After commit b2b49ccbdd54 (PM: Kconfig: Set PM_RUNTIME if PM_SLEEP is
selected) PM_RUNTIME is always set if PM is set, so #ifdef blocks
depending on CONFIG_PM_RUNTIME within #ifdef blocks depending on
CONFIG_PM may be dropped now.
Do that in drivers/power/pm2301_charger.c.

The ec_remove_handlers() is invoked without checking
EC_FLAGS_HANDLERS_INSTALLED, this patch enhances this check to avoid issues
that acpi_disable_gpe() is invoked unexpectedly to reduce the GPE runtime
count. This may happen when the EC handler installation failed on some
platforms.

When ring buffer returns an error indicating retry, storvsc may not
return a proper error code to SCSI when bounce buffer is not used.
This has introduced I/O freeze on RAID running atop storvsc devices.
This patch fixes it by always returning a proper error code.

Kernel build tools pointed out a memory leak so that has been
fixed and its error paths strengthened with a goto. Testing
showed compare and write was only working for lba=0; correcting
the length of the LBA field fixed that.

When we have an rport disconnect we race during rport deletion and
re-connection resulting in a panic. When we do this, we call
fc_remote_port_del() just before we do the calls to re-establish the
session with the FC transport with fc_remote_port_add() and then
fc_remote_port_rolechg().
If we remove the call to fc_remote_port_del() before re-establishing
the connection this prevents the race. This patch has resolved this
for multiple customers via test kernels.

This fixes random memory corruption triggered when all three of the
following are true:
* scsi-mq enabled
* T10 Protection Information (DIF) enabled
* SCSI host with sg_tablesize > SCSI_MAX_SG_SEGMENTS (128)
The symptoms of this bug are unpredictable memory corruption, BUG()s,
oopses, lockups, etc., any of which may appear to be completely
unrelated to the root cause.

Deep idle states like sleep and winkle are per core idle states. A core
enters these states only when all the threads enter either the
particular idle state or a deeper one. There are tasks like fastsleep
hardware bug workaround and hypervisor core state save which have to be
done only by the last thread of the core entering deep idle state and
similarly tasks like timebase resync, hypervisor core register restore
that have to be done only by the first thread waking up from these
state.
The current idle state management does not have a way to distinguish the
first/last thread of the core waking/entering idle states. Tasks like
timebase resync are done for all the threads. This is not only is
suboptimal, but can cause functionality issues when subcores and kvm is
involved.
This patch adds the necessary infrastructure to track idle states of
threads in a per-core structure. It uses this info to perform tasks like
fastsleep workaround and timebase resync only once per core.

The secondary threads should enter deep idle states so as to gain maximum
powersavings when the entire core is offline. To do so the offline path
must be made aware of the available deepest idle state. Hence probe the
device tree for the possible idle states in powernv core code and
expose the deepest idle state through flags.
Since the device tree is probed by the cpuidle driver as well, move
the parameters required to discover the idle states into an appropriate
common place to both the driver and the powernv core code.
Another point is that fastsleep idle state may require workarounds in
the kernel to function properly. This workaround is introduced in the
subsequent patches. However neither the cpuidle driver or the hotplug
path need be bothered about this workaround.
They will be taken care of by the core powernv code.

Pull take two of the GPIO updates:
"Same stuff as last time, now with a fixup patch for the previous
compile error plus I ran a few extra rounds of compile-testing.
This is the bulk of GPIO changes for the v3.19 series:
- A new API that allows setting more than one GPIO at the time. This
is implemented for the new descriptor-based API only and makes it
possible to e.g. toggle a clock and data line at the same time, if
the hardware can do this with a single register write. Both
consumers and drivers need new calls, and the core will fall back
to driving individual lines where needed. Implemented for the
MPC8xxx driver initially
- Patched the mdio-mux-gpio and the serial mctrl driver that drives
modems to use the new multiple-setting API to set several signals
simultaneously
- Get rid of the global GPIO descriptor array, and instead allocate
descriptors dynamically for each GPIO on a certain GPIO chip. This
moves us closer to getting rid of the limitation of using the
global, static GPIO numberspace
