git://git.kernel.org/pub/scm/linux/kernel/git/borntraeger/linux
Pull ACCESS_ONCE cleanup preparation from Christian Borntraeger:
"kernel: Provide READ_ONCE and ASSIGN_ONCE
As discussed on LKML http://marc.info/?i=54611D86.4040306%40de.ibm.com
ACCESS_ONCE might fail with specific compilers for non-scalar
accesses.
Here is a set of patches to tackle that problem.
The first patch introduce READ_ONCE and ASSIGN_ONCE. If the data
structure is larger than the machine word size memcpy is used and a
warning is emitted. The next patches fix up several in-tree users of
ACCESS_ONCE on non-scalar types.
This does not yet contain a patch that forces ACCESS_ONCE to work only
on scalar types. This is targetted for the next merge window as Linux
next already contains new offenders regarding ACCESS_ONCE vs.
non-scalar types"

strcmp was lost when all other string functions were removed,
but we still have an optimized version for this on CRISv32,
so any driver built as a module would not have access to this symbol.
In a similar manner, we had optimized versions of
csum_partial_copy_from_user and __do_clear_user
but no exported symbols for them, breaking bunch of other drivers
when built as a module.
At the same time, move EXPORT_SYMBOL(__copy_user) and

Pull x86 apic updates from Thomas Gleixner:
"After stopping the full x86/apic branch, I took some time to go
through the first block of patches again, which are mostly cleanups
and preparatory work for the irqdomain conversion and ioapic hotplug
support.
Unfortunaly one of the real problematic commits was right at the
beginning, so I rebased this portion of the pending patches without
the offenders.
It would be great to get this into 3.19. That makes reworking the
problematic parts simpler. The usual tip testing did not unearth any
issues and it is fully bisectible now.
I'm pretty confident that this wont affect the calmness of the xmas
season.
Changes:
- Split the convoluted io_apic.c code into domain specific parts
(vector, ioapic, msi, htirq)
- Introduce proper helper functions to retrieve irq specific data
instead of open coded dereferencing of pointers
- Preparatory work for ioapic hotplug and irqdomain conversion
- Removal of the non functional pci-ioapic driver
- Removal of unused irq entry stubs
- Make native_smp_prepare_cpus() preemtible to avoid GFP_ATOMIC

commit 4dbd27711cd9 "tick: export nohz tick idle symbols for module
use" was merged via the thermal tree without an explicit ack from the
relevant maintainers.
The exports are abused by the intel powerclamp driver which implements
a fake idle state from a sched FIFO task. This causes all kinds of
wreckage in the NOHZ core code which rightfully assumes that
tick_nohz_idle_enter/exit() are only called from the idle task itself.
Recent changes in the NOHZ core lead to a failure of the powerclamp
driver and now people try to hack completely broken and backwards
workarounds into the NOHZ core code. This is completely unacceptable
and just papers over the real problem. There are way more subtle
issues lurking around the corner.
The real solution is to fix the powerclamp driver by rewriting it with
a sane concept, but that's beyond the scope of this.
So the only solution for now is to remove the calls into the core NOHZ
code from the powerclamp trainwreck along with the exports.

Commit 85c116a6cb91 ("perf callchain: Make get_srcline fall back to sym+offset")
introduces asprintf() call and matches '%ld' to a u64 argument, which is
incorrect on ARM:

"These are regression fixes (leds-gpio, ACPI backlight driver,
operating performance points library, ACPI device enumeration
messages, cpupower tool), other bug fixes (ACPI EC driver, ACPI device
PM), some cleanups in the operating performance points (OPP)
framework, continuation of CONFIG_PM_RUNTIME elimination, a couple of
minor intel_pstate driver changes, a new MAINTAINERS entry for it and
an ACPI fan driver change needed for better support of thermal
management in user space.
Specifics:
- Fix a regression in leds-gpio introduced by a recent commit that
inadvertently changed the name of one of the properties used by the
driver (Fabio Estevam).
- Fix a regression in the ACPI backlight driver introduced by a
recent fix that missed one special case that had to be taken into
account (Aaron Lu).
- Drop the level of some new kernel messages from the ACPI core
introduced by a recent commit to KERN_DEBUG which they should have
used from the start and drop some other unuseful KERN_ERR messages
printed by ACPI (Rafael J Wysocki).
- Revert an incorrect commit modifying the cpupower tool (Prarit
Bhargava).
- Fix two regressions introduced by recent commits in the OPP library
and clean up some existing minor issues in that code (Viresh
Kumar).
- Continue to replace CONFIG_PM_RUNTIME with CONFIG_PM throughout the
tree (or drop it where that can be done) in order to make it
possible to eliminate CONFIG_PM_RUNTIME (Rafael J Wysocki, Ulf
Hansson, Ludovic Desroches).
There will be one more "CONFIG_PM_RUNTIME removal" batch after this
one, because some new uses of it have been introduced during the
current merge window, but that should be sufficient to finally get
rid of it.
- Make the ACPI EC driver more robust against race conditions related
to GPE handler installation failures (Lv Zheng).
- Prevent the ACPI device PM core code from attempting to disable
GPEs that it has not enabled which confuses ACPICA and makes it
report errors unnecessarily (Rafael J Wysocki).
- Add a "force" command line switch to the intel_pstate driver to
make it possible to override the blacklisting of some systems in
that driver if needed (Ethan Zhao).
- Improve intel_pstate code documentation and add a MAINTAINERS entry
for it (Kristen Carlson Accardi).
- Make the ACPI fan driver create cooling device interfaces witn
names that reflect the IDs of the ACPI device objects they are
associated with, except for "generic" ACPI fans (PNP ID "PNP0C0B").
That's necessary for user space thermal management tools to be able
to connect the fans with the parts of the system they are supposed
to be cooling properly. From Srinivas Pandruvada"

Currently functions in zsmalloc.c does not arranged in a readable and
reasonable sequence. With the more and more functions added, we may
meet below inconvenience. For example:
Current functions:
void zs_init()
{
}
static void get_maxobj_per_zspage()
{
}
Then I want to add a func_1() which is called from zs_init(), and this
new added function func_1() will used get_maxobj_per_zspage() which is
defined below zs_init().
void func_1()
{
get_maxobj_per_zspage()
}
void zs_init()
{
func_1()
}
static void get_maxobj_per_zspage()
{
}
This will cause compiling issue. So we must add a declaration:
static void get_maxobj_per_zspage();
before func_1() if we do not put get_maxobj_per_zspage() before
func_1().
In addition, puting module_[init|exit] functions at the bottom of the
file conforms to our habit.
So, this patch ajusts function sequence as:
/* helper functions */
...
obj_location_to_handle()
...
/* Some exported functions */
...
zs_map_object()
zs_unmap_object()
zs_malloc()
zs_free()
zs_init()
zs_exit()

For buffer write, page lock will be got in write_begin and released in
write_end, in ocfs2_write_end_nolock(), before it unlock the page in
ocfs2_free_write_ctxt(), it calls ocfs2_run_deallocs(), this will ask
for the read lock of journal->j_trans_barrier. Holding page lock and
ask for journal->j_trans_barrier breaks the locking order.
This will cause a deadlock with journal commit threads, ocfs2cmt will
get write lock of journal->j_trans_barrier first, then it wakes up
kjournald2 to do the commit work, at last it waits until done. To
commit journal, kjournald2 needs flushing data first, it needs get the
cache page lock.
Since some ocfs2 cluster locks are holding by write process, this
deadlock may hung the whole cluster.
unlock pages before ocfs2_run_deallocs() can fix the locking order, also
put unlock before ocfs2_commit_trans() to make page lock is unlocked
before j_trans_barrier to preserve unlocking order.

When running ocfs2 test suite multiple nodes reflink stress test, for a
4 nodes cluster, every unlink() for refcounted file needs about 700s.
The slow unlink is caused by the contention of refcount tree lock since
all nodes are unlink files using the same refcount tree. When the
unlinking file have many extents(over 1600 in our test), most of the
extents has refcounted flag set. In ocfs2_commit_truncate(), it will
execute the following call trace for every extents. This means it needs
get and released refcount tree lock about 1600 times. And when several
nodes are do this at the same time, the performance will be very low.
ocfs2_remove_btree_range()
-- ocfs2_lock_refcount_tree()
---- ocfs2_refcount_lock()
------ __ocfs2_cluster_lock()
ocfs2_refcount_lock() is costly, move it to ocfs2_commit_truncate() to
do lock/unlock once can improve a lot performance.

CMA size declared, there is no way to tell, other than the dmesg or
/var/log/messages logs.
With this patch we are showing the CMA info as part of meminfo, so that it
can be determined at any point of time. This will be populated only when
CMA is enabled.
Below is the sample output from a ARM based device with RAM:512MB and CMA:16MB.
MemTotal: 471172 kB
MemFree: 111712 kB
MemAvailable: 271172 kB

CmaTotal: 16384 kB
CmaFree: 6144 kB

When the system boots up, in the dmesg logs we can see the memory
statistics along with total reserved as below. Memory: 458840k/458840k
available, 65448k reserved, 0K highmem
When CMA is enabled, still the total reserved memory remains the same.
However, the CMA memory is not considered as reserved. But, when we see
/proc/meminfo, the CMA memory is part of free memory. This creates
confusion. This patch corrects the problem by properly subtracting the
CMA reserved memory from the total reserved memory in dmesg logs.
Below is the dmesg snapshot from an arm based device with 512MB RAM and
12MB single CMA region.
Before this change:
Memory: 458840k/458840k available, 65448k reserved, 0K highmem
After this change:
Memory: 458840k/458840k available, 53160k reserved, 12288k cma-reserved, 0K highmem

Longname is not correctly handled by hfsplus driver. If an attempt to
create a longname(>255) file/directory is made, it succeeds by creating a
file/directory with HFSPLUS_MAX_STRLEN and incorrect catalog key. Thus
leaving the volume in an inconsistent state. This patch fixes this issue.
Although lookup is always called first to create a negative entry, so just
doing a check in lookup would probably fix this issue. I choose to
propagate error to other iops as well.
Please NOTE: I have factored out hfsplus_cat_build_key_with_cnid from
hfsplus_cat_build_key, to avoid unncessary branching.
Thanks a lot.

This patch changes core_scsi3_pro_release() logic to allow an
existing AllRegistrants type reservation to be re-reserved by
any registered I_T nexus.
This addresses a issue where AllRegistrants type RESERVE was
receiving RESERVATION_CONFLICT status if dev_pr_res_holder did
not match the same I_T nexus, instead of just returning GOOD
status following spc4r34 Section 5.9.9:
"If the device server receives a PERSISTENT RESERVE OUT command
with RESERVE service action where the TYPE field and the SCOPE
field contain the same values as the existing type and scope
from a persistent reservation holder, it shall not make any
change to the existing persistent reservation and shall complete
the command with GOOD status."

While reviewing the code of umount_tree I realized that when we append
to a preexisting unmounted list we do not change pprev of the former
first item in the list.
Which means later in namespace_unlock hlist_del_init(&mnt->mnt_hash) on
the former first item of the list will stomp unmounted.first leaving
it set to some random mount point which we are likely to free soon.
This isn't likely to hit, but if it does I don't know how anyone could
track it down.
[ This happened because we don't have all the same operations for
hlist's as we do for normal doubly-linked lists. In particular,
list_splice() is easy on our standard doubly-linked lists, while
hlist_splice() doesn't exist and needs both start/end entries of the
hlist. And commit 38129a13e6e7 incorrectly open-coded that missing
hlist_splice().
We should think about making these kinds of "mindless" conversions
easier to get right by adding the missing hlist helpers - Linus ]

Commit bc96f648df1bbc2729abbb84513cf4f64273a1f1 (xen-netback: make
feature-rx-notify mandatory) incorrectly assumed that there were no
frontends in use that did not support this feature. But the frontend
driver in MiniOS does not and since this is used by (qemu) stubdoms,
these stopped working.
Netback sort of works as-is in this mode except:
- If there are no Rx requests and the internal Rx queue fills, only
the drain timeout will wake the thread. The default drain timeout
of 10 s would give unacceptable pauses.
- If an Rx stall was detected and the internal Rx queue is drained,
then the Rx thread would never wake.
Handle these two cases (when feature-rx-notify is disabled) by:
- Reducing the drain timeout to 30 ms.
- Disabling Rx stall detection.

Load balancing can be triggered in the critical sections protected by
srmmu_context_spinlock in destroy_context() and switch_mm() and can hang
the cpu waiting for the rq lock of another cpu that in turn has called
switch_mm hangning on srmmu_context_spinlock leading to deadlock.
So, disable interrupt while taking srmmu_context_spinlock in

Currently, searching for a socket to add a reference to is not
synchronized with deletion of sockets. This can result in use
after free if there is another operation that is removing a
socket at the same time. Solving this requires both holding the
appropriate lock and checking the refcount to ensure that it
has not already hit zero.

Each mmap Netlink frame contains a status field which indicates
whether the frame is unused, reserved, contains data or needs to
be skipped. Both loads and stores may not be reordeded and must
complete before the status field is changed and another CPU might
pick up the frame for use. Use an smp_mb() to cover needs of both
types of callers to netlink_set_status(), callers which have been
reading data frame from the frame, and callers which have been
filling or releasing and thus writing to the frame.
- Example code path requiring a smp_rmb():
memcpy(skb->data, (void *)hdr + NL_MMAP_HDRLEN, hdr->nm_len);
netlink_set_status(hdr, NL_MMAP_STATUS_UNUSED);

The Arcam rPAC seems to have the same problem - whenever anything
(alsamixer, udevd, 3.9+ kernel from 60af3d037eb8c, ..) attempts to
access mixer / control interface of the card, the firmware "locks up"
the entire device, resulting in
SNDRV_PCM_IOCTL_HW_PARAMS failed (-5): Input/output error
from alsa-lib.
Other operating systems can somehow read the mixer (there seems to be
playback volume/mute), but any manipulation is ignored by the device
(which has hardware volume controls).

It turns out that there's a lurking ABI issue. GCC, when
compiling this in a 32-bit program:
struct user_desc desc = {
.entry_number = idx,
.base_addr = base,
.limit = 0xfffff,
.seg_32bit = 1,
.contents = 0, /* Data, grow-up */
.read_exec_only = 0,
.limit_in_pages = 1,
.seg_not_present = 0,
.useable = 0,
};
will leave .lm uninitialized. This means that anything in the
kernel that reads user_desc.lm for 32-bit tasks is unreliable.
Revert the .lm check in set_thread_area(). The value never did
anything in the first place.

Let's change the code to access the page table elements with
READ_ONCE that does implicit scalar accesses for the gup code.
mm_find_pmd is tricky, because m68k and sparc(32bit) define pmd_t
as array of longs. This code requires just that the pmd_present

is sufficent.
A similar case is in handle_pte_fault. On ppc44x the word size is
32 bit, but a pte is 64 bit. A barrier is ok as well.

At the moment, if p and x are both of the same bitwise type
(eg. __le32), get_user(x, p) produces a sparse warning.
This is because *p is loaded into a long then cast back to typeof(*p).
When typeof(*p) is a bitwise type (which is uncommon), such a cast needs
__force, otherwise sparse produces a warning.
For non-bitwise types __force should have no effect, and should not hide
any legitimate errors.
Note that we are casting to typeof(*p) not typeof(x). Even with the
cast, if x and *p are of different types we should get the warning, so I
think we are not loosing the ability to detect any actual errors.
virtio would like to use bitwise types with get_user() so fix these
spurious warnings by adding __force.

still be changed at runtime and updated. Drivers using a 0444 permission

For drivers that want to allow updates, any S_IW* flag will set the

git://git.kernel.org/pub/scm/linux/kernel/git/ebiederm/user-namespace
Pull user namespace related fixes from Eric Biederman:
"As these are bug fixes almost all of thes changes are marked for
backporting to stable.
The first change (implicitly adding MNT_NODEV on remount) addresses a
regression that was created when security issues with unprivileged
remount were closed. I go on to update the remount test to make it
easy to detect if this issue reoccurs.
Then there are a handful of mount and umount related fixes.
Then half of the changes deal with the a recently discovered design
bug in the permission checks of gid_map. Unix since the beginning has
allowed setting group permissions on files to less than the user and
other permissions (aka ---rwx---rwx). As the unix permission checks
stop as soon as a group matches, and setgroups allows setting groups
that can not later be dropped, results in a situtation where it is
possible to legitimately use a group to assign fewer privileges to a
process. Which means dropping a group can increase a processes
privileges.
The fix I have adopted is that gid_map is now no longer writable
without privilege unless the new file /proc/self/setgroups has been
set to permanently disable setgroups.
The bulk of user namespace using applications even the applications
using applications using user namespaces without privilege remain
unaffected by this change. Unfortunately this ix breaks a couple user
space applications, that were relying on the problematic behavior (one
of which was tools/selftests/mount/unprivileged-remount-test.c).
To hopefully prevent needing a regression fix on top of my security
fix I rounded folks who work with the container implementations mostly
like to be affected and encouraged them to test the changes.

The optimization for filtering out extended inquiry results, advertising
reports or scan response data based on provided UUID list has a logic
bug. In case no match is found in the advertising data, the scan
response is ignored and not checked against the filter. This will lead
to events being filtered wrongly.
Change the code to actually only drop the events when the scan response
data is not present. If it is present, it needs to be checked against
the provided filter.
The patch is a bit more complex than it needs to be. That is because
it also fixes this compiler warning that some gcc versions produce.

Dave Hansen reports that commit fb7332a9fedf ("mmu_gather: move minimal
range calculations into generic code") caused a performance problem:
"tlb_finish_mmu() goes up about 9x in the profiles (~0.4%->3.6%) and
tlb_flush_mmu_free() takes about 3.1% of CPU time with the patch
applied, but does not show up at all on the commit before"
and the reason is that Will moved the test for whether we need to flush
from tlb_flush_mmu() into tlb_flush_mmu_tlbonly(). But that meant that
tlb_flush_mmu_free() basically lost that check.
Move it back into tlb_flush_mmu() where it belongs, so that it covers

DMA is opt-in for this driver. So, we can't use deferred probing for
requesting DMA channels in probe, because our driver would get endlessly
deferred if DMA support is compiled in AND the DMA driver is missing.
Because we can't know when the DMA driver might show up, we always try
again when a DMA transfer would be possible. The downside is that there
is more overhead for setting up PIO transfers under the above scenario.
But well, having DMA enabled and the proper DMA driver missing looks
like a broken or test config anyhow.

Originally, the I2C controller supported by the i2c-mv64xxx driver
requires a lot of software support: an interrupt is generated at each
step of an I2C transaction (after the start bit, after sending the
address, etc.) and the driver is in charge of re-programming the I2C
controller to do the next step of the I2C transaction. This explains
the fairly complex state machine that the driver has.
On Marvell Armada XP and later processors (Armada 375, 38x, etc.), the
I2C controller was extended with a part called the "I2C Bridge", which
allows to offload the I2C transaction completely to the
hardware. Initial support for this mechanism was added in commit
930ab3d403a ("i2c: mv64xxx: Add I2C Transaction Generator support").
However, the implementation done in this commit has two related
issues, which this commit fixes by completely changing how the offload
implementation is done:
* SMBus read transfers, where there is one write to select the
register immediately followed in the same transaction by one read,
were making the processor hang. This was easier visible on the
Marvell Armada XP WRT1900AC platform using a driver for an I2C LED
controller, or on other Armada XP platforms by using a simple
'i2cget' command to read an I2C EEPROM.
* The implementation was based on the fact that the offload engine
was re-programmed to transfer each message of an I2C xfer: this
meant that each message sent with the offload engine was starting
with a normal I2C start sequence. However, the I2C subsystem
assumes that all messages belonging to the same xfer will use the
so-called "repeated start" so that the entire I2C xfer is seen as
one transfer by the I2C devices and cannot be interrupt by other
I2C masters on the same bus.
In fact, the "I2C Bridge" allows to offload three types of xfer:
- xfer of one write message
- xfer of one read message
- xfer of one write message followed by one read message
For all other situations, we have to fallback to not using the "I2C
Bridge" in order to get proper I2C semantics.
Therefore, this commit reworks the offload implementation to put it
not at the message level, but at the xfer level: in the
mv64xxx_i2c_xfer() function, we decide if the transaction can be
offloaded (in which case it is handled by the
mv64xxx_i2c_offload_xfer() function), or otherwise it is handled by
the slow path (implemented in the existing mv64xxx_i2c_execute_msg()).
This allows to simplify the state machine, which no longer needs to
have any state related to the offload implementation: the offload
implementation is now completely separated from the slow path (with
the exception of the interrupt handler, of course).
In summary:
- mv64xxx_i2c_can_offload() will analyze an I2C xfer and decided of
the "I2C Bridge" can be used to offload it or not.
- mv64xxx_i2c_offload_xfer() will actually program the "I2C Bridge"
to offload one xfer (of either one or two messages), and block
using mv64xxx_i2c_wait_for_completion() until the xfer completes.
- The interrupt handler mv64xxx_i2c_intr() is modified to push the
offload related code to a separate function,
mv64xxx_i2c_intr_offload(). It will take care of reading the

After converting inline data to normal data, client need to flush
the new i_inline_version (CEPH_INLINE_NONE) to MDS. This commit makes

Before any data write, convert inline data to normal data and set

empty snap realm to another empty snap realm. After changing inode's
snap realm, some dirty pages' snap context can be not equal to inode's

After creating/deleting/renaming file, offsets of sibling dentries may
change. So we can not use cached dentries to satisfy readdir. But we can
still use the cached dentries to conclude -ENOENT for lookup.
This patch introduces a new inode flag indicating if child dentries are
ordered. The flag is set at the same time marking a directory complete.
After creating/deleting/renaming file, we clear the flag on directory
inode. This prevents ceph_readdir() from using cached dentries to satisfy
readdir syscall.

When a lock operation is interrupted, current code sends a unlock request to
MDS to undo the lock operation. This method does not work as expected because
the unlock request can drop locks that have already been acquired.
The fix is use the newly introduced CEPH_LOCK_FCNTL_INTR/CEPH_LOCK_FLOCK_INTR

locks that have alread been acquired, they only interrupt blocked file lock
request.

domain being gated
Audio subsystem clocks are located in separate block. On Exynos 5420 if
clock for this block (from main clock domain) 'mau_epll' is gated then
any read or write to audss registers will block.
This kind of boot hang was observed on Arndale Octa and Peach Pi/Pit
after introducing runtime PM to pl330 DMA driver. After that commit the
'mau_epll' was gated, because the "amba" clock was disabled and there
were no more users of mau_epll.
The system hang on one of steps:
1. Disabling unused clocks from audss block.
2. During audss GPIO setup (just before probing i2s0 because
samsung_pinmux_setup() tried to access memory from audss block which was
gated.
Add a workaround for this by enabling the 'mau_epll' clock in probe.

As we already show mountpoints relative to the root directory, thanks
to the change made back in 2000, change show_vfsmnt() and show_vfsstat()
to skip out-of-root mountpoints the same way as show_mountinfo() does.

Starting with commit v3.2-rc4-1-g02125a8, seq_path_root() no longer
changes the value of its "struct path *root" argument.
Starting with commit v3.2-rc7-104-g8c9379e, the "struct path *root"
argument of seq_path_root() is const.
As result, the temporary variable "root" in show_mountinfo() that
holds a copy of struct path root is no longer needed.

If mount flags don't have MS_RDONLY, iso9660 returns EACCES without actually
checking if it's an iso image.
This tricks mount_block_root() into retrying with MS_RDONLY. This results
in a read-only root despite the "rw" boot parameter if the actual
filesystem was checked after iso9660.
I believe the behavior of iso9660 is okay, while that of mount_block_root()
is not. It should rather try all types without MS_RDONLY and only then
retry with MS_RDONLY.
This change also makes the code more robust against the case when EACCES is
returned despite MS_RDONLY, which would've resulted in a lockup.

Restore the correct checks, and move NUL-termination into scanarg(),
while we are at it.
Incidentally, mixing "coding style cleanups" (for small values of cleanup)
with functional changes is a Bad Idea(tm)...

When writing the code to allow per-station GTKs, I neglected to
take into account the management frame keys (index 4 and 5) when
freeing the station and only added code to free the first four
data frame keys.
Fix this by iterating the array of keys over the right length.

Currently the H_CONFER hcall is implemented in kernel virtual mode,
meaning that whenever a guest thread does an H_CONFER, all the threads
in that virtual core have to exit the guest. This is bad for
performance because it interrupts the other threads even if they
are doing useful work.
The H_CONFER hcall is called by a guest VCPU when it is spinning on a
spinlock and it detects that the spinlock is held by a guest VCPU that
is currently not running on a physical CPU. The idea is to give this
VCPU's time slice to the holder VCPU so that it can make progress
towards releasing the lock.
To avoid having the other threads exit the guest unnecessarily,
we add a real-mode implementation of H_CONFER that checks whether
the other threads are doing anything. If all the other threads
are idle (i.e. in H_CEDE) or trying to confer (i.e. in H_CONFER),
it returns H_TOO_HARD which causes a guest exit and allows the
H_CONFER to be handled in virtual mode.
Otherwise it spins for a short time (up to 10 microseconds) to give
other threads the chance to observe that this thread is trying to
confer. The spin loop also terminates when any thread exits the guest
or when all other threads are idle or trying to confer. If the
timeout is reached, the H_CONFER returns H_SUCCESS. In this case the
guest VCPU will recheck the spinlock word and most likely call
H_CONFER again.
This also improves the implementation of the H_CONFER virtual mode
handler. If the VCPU is part of a virtual core (vcore) which is
runnable, there will be a 'runner' VCPU which has taken responsibility
for running the vcore. In this case we yield to the runner VCPU
rather than the target VCPU.
We also introduce a check on the target VCPU's yield count: if it
differs from the yield count passed to H_CONFER, the target VCPU
has run since H_CONFER was called and may have already released
the lock. This check is required by PAPR.

There are two ways in which a guest instruction can be obtained from
the guest in the guest exit code in book3s_hv_rmhandlers.S. If the
exit was caused by a Hypervisor Emulation interrupt (i.e. an illegal
instruction), the offending instruction is in the HEIR register
(Hypervisor Emulation Instruction Register). If the exit was caused
by a load or store to an emulated MMIO device, we load the instruction
from the guest by turning data relocation on and loading the instruction
with an lwz instruction.
Unfortunately, in the case where the guest has opposite endianness to
the host, these two methods give results of different endianness, but
both get put into vcpu->arch.last_inst. The HEIR value has been loaded
using guest endianness, whereas the lwz will load the instruction using
host endianness. The rest of the code that uses vcpu->arch.last_inst
assumes it was loaded using host endianness.
To fix this, we define a new vcpu field to store the HEIR value. Then,
in kvmppc_handle_exit_hv(), we transfer the value from this new field to
vcpu->arch.last_inst, doing a byte-swap if the guest and host endianness
differ.

This removes the code that was added to enable HV KVM to work
on PPC970 processors. The PPC970 is an old CPU that doesn't
support virtualizing guest memory. Removing PPC970 support also
lets us remove the code for allocating and managing contiguous
real-mode areas, the code for the !kvm->arch.using_mmu_notifiers
case, the code for pinning pages of guest memory when first
accessed and keeping track of which pages have been pinned, and
the code for handling H_ENTER hypercalls in virtual mode.
Book3S HV KVM is now supported only on POWER7 and POWER8 processors.
The KVM_CAP_PPC_RMA capability now always returns 0.

Currently the calculations of stolen time for PPC Book3S HV guests
uses fields in both the vcpu struct and the kvmppc_vcore struct. The
fields in the kvmppc_vcore struct are protected by the
vcpu->arch.tbacct_lock of the vcpu that has taken responsibility for
running the virtual core. This works correctly but confuses lockdep,
because it sees that the code takes the tbacct_lock for a vcpu in
kvmppc_remove_runnable() and then takes another vcpu's tbacct_lock in
vcore_stolen_time(), and it thinks there is a possibility of deadlock,
causing it to print reports like this:

In order to make the locking easier to analyse, we change the code to
use a spinlock in the kvmppc_vcore struct to protect the stolen_tb and
preempt_tb fields. This lock needs to be an irq-safe lock since it is
used in the kvmppc_core_vcpu_load_hv() and kvmppc_core_vcpu_put_hv()
functions, which are called with the scheduler rq lock held, which is
an irq-safe lock.

When running in non-cache coherent configuration the memory that was
allocated with dma_alloc_coherent() has a custom mapping and so there is no
1-to-1 relationship between the kernel virtual address and the PFN. This
means that virt_to_pfn() will not work correctly for those addresses and the
default mmap implementation in the form of dma_common_mmap() will map some
random, but not the requested, memory area.
Fix this by providing a custom mmap implementation that looks up the PFN
from the page table rather than using virt_to_pfn.
